Part X, line 2 The Trust recognizes and measures tax positions take or expected to be taken its tax return based on their technical merit and assesses the likelihood that the positions will be sustained upon examination based on the facts circumstances and information available at the end of each period. Interest and penalties on tax liabilities if any would be recorded in interest expense.
Part XI, line 2d The difference in revenue is a result of the fact that the Form 990 is prepared on a calendar year basis and the audited financial statements have a June 30 year end.

Part VI, Line 6 The South Florida Physicians Excess Insuring Trust is a malpractice Self Insurance Trust providing excess additional malpractice coverage to members of the South Florida Ophthalmological Self Insuring Trust. Membership is limited to Ophthalmologists who are duly licensed by the State of Florida. All applications for membership are made on a written application form provided by the Board of Trustees. Approval for membership by the Board of Trustees requires a vote of the Board.
Part VI, Line 7a The affairs of the South Florida Physicians Excess Insuring Trust is managed and its corporate powers exercised by a Board of Trustees. Currently there are 3 members. At the annual membership meeting outgoing trustees shall act as a nominating committee and shall submit to the members a slate of Trustees to fill the offices which are open each year.
Part VI, Line 11a Once the Form 990 has been finalized by the accountant it is reviewed and approved and filed by the South Florida Physicians Excess Insuring Trust.
