Form 990, Part VI, Section A, line 3	The Trust has contracted with third-party administrators (Vimly Benefit Solutions, Inc. and Associated Industries Management Services) to provide bookkeeping and administrative services.
Form 990, Part VI, Section A, line 7a	Trustees are appointed by the Trust Administrative Committee. The Trust Administrative Committee may terminate a trustee. In the event of the termination or resignation of a trustee, a successor trustee will be appointed by the Trust Administrative Committee. The Trust Administrative Committee will notify participating employers of the appointment of a successor trustee. The trustee will consent to and accept appointment in writing.
Form 990, Part VI, Section B, line 11b	The Form 990 was prepared under the guidance of the Trustee by the independent accounting firm SCHOEDEL & SCHOEDEL, Certified Public Accountants, PLLC.
