THIS RETURN WAS EXTENED BUT FAILED TO MEET EXTENDED DUE DATE DUE TO COMBINATION OF HEATH ISSUES AND EMAIL ADDRESS WAS DISCONTINUED BY PROVIDER CAUSING INABILITY TO COMPLETE RETURN TIMELY. THIS IS FIRST TIME EVER BEING LATE SINCE INCEPTION AND WE ARE ASKING FOR ABATEMENT OF ANY PENALTIES BASED ON THIS BEING THE FIRST TIME EVER SINCE INCEPTION TO BE LATE.
