Explanation:

The organization's leaders were relying on an accountant to prepare and electronically file the 2021 Form 990-EZ. This accountant told the organization's leaders that he had filed the return. However, it appears that he might not have. The preparer has subsequently died, and so there is no way to find out if he did file the return. The organization's leaders therefore filed a paper return, unaware that the return had to be filed electronically. It was sent back by the IRS with a letter dated August 31, 2022. An additional complication is the fact that the organization lost its tax-exempt status for failing to file three consecutive informational returns. It applied for and obtained retroactive reinstatement of its exempt status in a letter dated 11/21/2021. However, the IRS Select Check Database still does not show the organization as reinstated as an exempt organization. The organization's leaders believe that they exercised diligent efforts to timely file the 2021 return, and they ask that the IRS not charge any penalties for the late filing.
