Return Reference Explanation
Part I Line 1 The services of the orgainization were no longer needed by the Pierce County, Wisconsin, Circuit Court. Only 2 wards remained. They were transferred to other providers. Loberg Law Office provided services to the organization. The total outstanding bill, a running total from inception, was 33360.64. The remainng assets in checking of 4889 were applied to that bill and the balance written off.

Return Reference Explanation
Form 990, Part VI, Section A, Line 2 Robert L Loberg and Lars H Loberg are attorneys with Loberg Law Office, LLP. The law offices provides accounting and other services to the Company and the wards. Loberg Law Office was requested to furnish Guardianship services to those who did not have family or other persons available to provide such services. This request was made several years ago by the Circuit Judge. Recently the number of wards declined to only two. Therefore it was decided to transfer the wards to other non-profit providers and disband this entity.
Form 990, Part VI, Section B, Line 12c Monitoring by directors who are husband, wife and son. As noted only two wards were being served and their supervision was transferred to other providers and approved by the Circuit Court.
