    Statement:
    The exempt status determination approval letter from IRS was never received. The information on date of approval was obtained through a phone call. The 2020 tax return 990 was sent by mail. IRS never returned the form or sent any subsequent letters. In May of 2022, the tax return was filed by mail after many attempts to e-file. The e-file kept getting rejected because the EIN had not . been established as an exempt entity. After many call and a request for information via fax, we were able to print the letter from an IRS not-for-profit corporation site. This is when we found out that the IRS had classified Chrysally Wellness Center, Inc. a private foundation. We dont understand why the classification and will be filing for a public charity classification. All Pertaining and required forms and notifications will be filed.
