    THE TAXPAYER HAS ROUTINELY MET FILING REQUIREMENTS FOR FORM 990-PF AND INFORMATION REPORTING ON A TIMELY BASIS. THE LATE FILING OF FORM 8868 AND FORM 990 FOR THE YEAR ENDED DECEMBER 31, 2021 AND FORM 8868 FOR THE YEAR ENDED DECEMBER 31, 2022 WERE AN IRREGULARITY DUE TO CLIENT HEALTH ISSUES AND AN INTERNAL ERROR IN THE RECORD KEEPING THE OUTSIDE CPA FIRM UTILIZES TO TRACK THE FILING NEEDS AND DEADLINES. THE LYCHE FAMILY FOUNDATION WAS STARTED BY JOHN AND LINDA LYCHE. DURING 2021 AND 2022 LINDA WAS ILL WITH ALS AND LATER PASSED AWAY. THE REPORTING REQUIREMENTS OF THE LYCHE FAMILY FOUNDATION WERE OVERLOOKED BY THE CLIENT AND THE ORGANIZATION RELIED UPON THEIR PAID PREPARER TO FILE THE PROPER EXTENSIONS TO EXTEND THE ORIGINAL DUE DATE OF THE RETURN AND FILE THE RETURN. HOWEVER, THE FILING OF FORM 990 AND FORMS 8868 WERE MISTAKENLY MISSED. ONCE THE ERROR WAS DISCOVERED THE RETURNS WERE FILED IMMEDIATELY. THE PAID PREPARER USES A COMBINATION OF TAX SOFTWARE AND FILE TRACKING SOFTWARE TO RECORD COMPLETION OF EXTENSIONS, SO GOING FORWARD THIS SHOULD NOT HAPPEN AGAIN.
