Explanation:

The person who e-filed the 2021 Form 990-EZ for the organization moved to a different state and was not available for consultation. The president of the organization, Kay Miller, believed it was okay to access the Internal Revenue Service website, fill-in the Form 990-EZ, print and mail the return. This is what she did on May 11, 2023. She was not aware that the return had to be e-filed. The organization has subsequently received a notice from the Internal Revenue Service that the mailed return could not be processed. The return is now being e-filed. We are asking that you waive any penalty that may be payable due to not e-filing the return. The president of the organization did what she thought was correct. Thank you for your assistance in this matter.
