Explanation:

COVID 19 Illness/Death: We sent all info and payment to an accountant and received confirmation he had filed in February of 2022. Upon receiving notice from IRS I reached out to him and received word from his office that he had gotten ill with COVID in 2022 and passed. I genuinely thought it had been filed and never checked back in until IRS sent notice.
