Date: Oct 16th 2023 To whom it may concern I am responding to the IRS letter dated Sept 28th 2023 0422897266LTR 2697C received Oct 10th 2023. Thank you for notifying me that our 2021 990-PF forms have been returned and require correction. Attached with the return 990-PF was a 2021 e-file Activity Report which helped identify the missing 8453-TE Signature form.In addition to the 8453-TE form not being provided I have also noted a minor spelling error in the foundation name Debbie should be Debbi of which I will also correct. I believe this omission may also have been the reason for the original 990-PF rejection and inability to e-file upon a first IRS notice. As a consequence the second e-file submission of the 990-PF the form was returned by preparer by mail and also rejected. I have once again contacted my preparer to verify that the status of the 8453-TE form and if it was omitted in previous attempts. Although I have worked with this preparer for many years including my personal taxes based a disturbing trend of omissions an lack of responsivenessI have elected to resubmit by e-file the 990-PF form myself along with this letter of explanation. It was my understanding that the forms in question were prepared and processed by our tax advisory preparer and was not aware of any issue associated with the said filling until notified by the IRS. Rest assured it is our intention to comply with all appropriate rules and regulations associated with our foundation. To that end I have re-submitted the 2021 990-PF form by e-file and per my signature as an officer of the foundation.
