Form 990, Part VI, Line 4: Description of Significant Changes to Organizational Documents	The Bylaws were amended, effective June 23, 2022, to change membership from two classes of members, the voting members and the associate members, to just one class, the voting members.
Form 990, Part VI, Line 6: Explanation of Classes of Members or Shareholder	As of June 23, 2022, there shall be one class of members, the voting members. The voting members shall be any person or entity who contracts with the organization for employee benefits managed or handled by the organization.
Form 990, Part VI, Line 11b: Form 990 Review Process	The Form 990 is prepared by an independent public accounting firm. It is first reviewed by the executive director and the organization's controller. It is then provided to the finance committee for their review and comment. Any identified changes as a result of these reviews are made to the Form 990. A final copy is provided to the full board of directors prior to filing with the Internal Revenue Service.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts	Each year a conflict of interest compliance questionnaire is completed by all officers, directors and key employees. If any responses require attention, the executive director investigates the potential conflict and brings any issues to the board of directors for resolution.
Form 990, Part VI, Line 15a: Compensation Review & Approval Process - CEO, Top Management	The organization's compensation committee reviews annual salary surveys and the executive director's strategic objectives and accomplishments. The compensation committee then makes a recommendation to the full Board of directors for the Executive Director's annual salary. The board then votes to approve the amount of compensation to be paid. Contemporaneous written records are maintained to document these procedures.
Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees	No other officers receive compensation nor are there any key employees in the organization.
Form 990, Part VI, Line 19: Other Organization Documents Publicly Available	The organization does not make its governing documents, conflict of interest policy and financial statements available to the public.
