| Form 990, Part III, Line 4a PROGRAM SERVICE ACCOMPLISHMENTS | FISCAL YEAR END 2022 COMMUNITY CANCER RESEARCH FOUNDATION, INC. (CCRF) ACTIVITIES (July 1, 2021 through June 30, 2022) - The Cancer Research Foundation relies on public contributions to help offset the costs of conducting clinical trials and offering support services. Through financial and resource donations of many individuals and organizations, these services have been possible. In addition, the Community Healthcare System Hospitals provide financial, in-kind and personnel support for many of the activities of the Foundation. COMMUNITY SUPPORT: If not for community support, it would be difficult to continue to fulfill our mission. We are thankful for the many community groups and businesses who continue to support our organization and account for a large part of our income. We have had numerous individuals and organizations hold fundraisers for our benefit and we truly appreciate their selfless efforts. In addition, the Community Foundation of Northwest Indiana and a number of its entities contribute not only monetarily, but through the time and talents of many employees. FOUNDATION ACTIVITIES: The foundation continuously seeks opportunities to educate the public and professionals about the latest in cancer care from prevention and screening through survivorship. It also supports clinical trials in the community so individuals diagnosed with cancer do not always have to travel to distant hospitals to receive this type of treatment. The Administrator participated in Cancer Committee for Community Hospital, St. Catherine Hospital and St. Mary Medical Center. The administrator also participates in the Breast Leadership Committee both at Community Hospital and St, Mary Medical Center, Chemotherapy Committee, Research Committee at Community Hospital, Oncology Steering Committee at St. Mary's, Cancer Quality Committee and Cancer Care Leadership Committee. The Cancer Committees plan activities of the cancer program and help maintain and improve practice standards to meet the criteria outlined by the American College of Surgeons Commission on Cancer. The goal is to have continuity of care throughout the system for the cancer patient and for public education and awareness. CANCER RESEARCH DEPARTMENT ACTIVITIES/ACCOMPLISHMENTS 1. Navigation: Navigation Services continue to expand and restructure to better meet the needs of our patients and health system - Director of Cancer Care Services was hired to assist and oversee research, navigation, registry and the Cancer Resource Centre, including outreach activities - Strategic focus remains on breast and lung cancer navigation. Currently, a position for a new nurse navigator has been posted with the plan to expand our focus to GI cancers - Integrated the use of EON software and incidental lung/breast nodules into the navigation team workflows. EON software has helped identify many patients with early stage asymptomatic cancers - Expanded navigation from survivorship to the entire continuum between screening/diagnoses to survivorship; two nurse navigators were hired, one for breast and one for lung navigation - Hired Licensed Clinical Social Worker for Cancer Care Services to support patients with case management/social deterrents and barriers to care as well as psychological support. 2. Cancer Resource Centre/Outreach: The COVID-19 EPIC brought a halt to in-person activities at the Cancer Resource Centre March 13, 2020 through August 2021. The Cancer Resource Centre opened its doors to visitors in August 2021. - Due to Covid-19, multiple support groups were established on web-ex to support patients and care givers through their journey. - Cancer Survivor's Day - June 5, 2022 at Community Stroke and Rehabilitation Center - 116 total participants. Very successful and most attendees since 2012. - A successful Lights of Life presentation and virtual tree lighting ceremony was held in December 2021. This is an annual presentation and is currently being planned for December 2022. - St. Catherine Cancer Support Group developed - Prostate Networking Group- Community Stroke & Rehabilitation Center - Reinstated 1:1 programs such as Chair Massage, Reiki and Reiki/Reflexology. - Physician Lectures with the option of virtual attendance - Oncology Rehabilitation Lecture - Power of Your Story- motivational lecture 3. Cancer Research : - Contracted with Carle Cancer Clinic to become a National Cancer Institute (NCI) affiliated site. This allows CHS access to more than 100 NCI clinical trials. - When contract was signed in January 2021, all NCI clinical trials were placed on hold as we transitioned into the new affiliation. Existing follow up patients were uploaded into the Carle Cancer Registry while FWA certificates, CITI training for providers, staff and nurses, COIs were signed, and the NCI credentialed and loaded all providers into their system under the new affiliation. - Completed all administration/regulatory requirements with Carle and opened our 1st clinical trial February 24, 2022. - New Research Manager started - 12/13/21. - Developed monthly Research Bulletin to help keep provider and staff abreast to current trials available. Also developed monthly research meeting to discuss patient care and potential new trials available. CANCER RESEARCH STUDIES ANNUAL ACCRUAL Fiscal Year End June 30, 2022 - Patients screened - 1331 - Patients enrolled - 21 - Patients followed - 62 - Open Protocols - 12 - Clinical trials closed to accrual from January 2021 to February 2022 in preparation of partnering with Carle Clinic Prescreen halted from June 2021 to March 2022 Starting February 24, 2022 to Jun 30, 2022: - 4 trials re-opened to accrual, 8 trials newly opened to accrual - 15 studies remained active as closed to accrual with patients in follow-up - 2 studies that were Re-Opened to accrual closed to accrual GROWTH INITIATIVES - NEW PROGRAMS/PROJECTS NAVIGATION: - Expand our nurse navigation team to include GI cancers - Adopted new navigation system (EON) to identify, track and follow incidental lung and breast nodules. Cancer Resource Centre / Outreach - Community Advisory Board - tentatively planned Fall 2022 - Cancer & Genetics Symposium -tentatively planned Fall 2022 Cancer Research - Increase patient accruals - Expand the numbers of trials open to accrual, including various disease sites and stages - Continue working with oncologist to help educate and increase awareness of available trials Supportive Care Model - Enhanced Genetic Program with established Genetic Counselor Adopting patient survey for investigative assessment of eligibility for genetic testing. Exploring two technological workflows to increase patient awareness and referrals for genetic counseling. - Expand Outpatient Palliative Care Services for the system Opened outpatient palliative care office January 2022; however due to low patient referrals and barriers to care/access office was closed April 2022. Determined need for more outpatient / in-home palliative care support. Plan to build stronger relationships with outpatient palliative care programs to ensure a strong referral process and continuity of care. Continue to educate patients, family and healthcare staff on the benefits of palliative care. - Oncology Rehabilitation It is offered by the rehabilitation specialists at all three hospitals; cancer patients are being referred in the early stages of diagnosis in preparation for symptom management, physical limitations and progress during the course of treatment with surgery, radiation and chemotherapy treatments. Cancer Registry / Data Reporting - Oncolog, Cancer Registry System adopted in July 2021. Utilize new software database to accurately and timely track cancer statistics for our health system, build reports and analyze data for future strategic growth decisions. - Implement synoptic reporting for surgeons on certain breast and colon surgical cases, per Commission on Cancer standards - Creating informational dashboard to track quality and growth with Data Analytics - in progress FUNDRAISING EVENTS Unite & Fight 2022: Major fundraiser for the Cancer Resource Centre. This event was canceled due to Covid -19 but in 2022 we are moving forward with this event. Tentatively planned for October 2, 2022. Lights of Life (December 2021): This event raised over $11,560 for cancer research.
