Explanation:

The return was filed late because of a miscommunication between the Treasurer and the return filer. They were both under the the assumption that an extension had been filed, when it was not. The error was discover when the preparer began to complete the return and realized no extension had been filed. This was a total oversight, not an attempt to avoid filing a return.
