Schedule R, Part II	The Lutheran Church-Missouri Synod is the sponsoring organization of the Concordia Disability and Survivor VEBA. At 12/31/22, there were 5,897 affiliated churches, schools, and related parties enrolled in the plan of which 3,904 made contributions for at least one employee to the plan.
Schedule R, Part V, Line 1n	The Lutheran Church-Missouri Synod is the sponsoring organization of the VEBA. Concordia Plan Services (CPS) administers the plans/trusts of the LCMS. Concordia Disability and Survivor Plan is one of the plans/trusts administered by CPS. All the plans/trusts operate within the same building and share resources.
