Part VI, Line 6	The Cooperative is organized as member or member owned corporation.
Part VI, Line 7a	Cooperative members can nominate other members to run for election to the governing board. Annually an election is held where members elect the board of directors.
Part VI, Line 7b	The sale of the Cooperative is reserved and subject to approval by the members of the Cooperative.
Part VI, Line 11b	A draft Form 990 is presented annually at a regular monthly board meeting for full Board review consideration and approval. Once the board is satisfied with the content of the form and voted on affirmatively the return is signed and filed.
Part VI, Line 12c	The Cooperative surveys each officer and director annually for possible conflicts of interest. The results of the survey are reviewed and considered by the Board of Directors during an official meeting.
Part VI, Line 15	The Cooperative's Board of Directors uses an industry specific compensation data for annual consideration and adjustment of the CEO's wage. The CEO also uses the industry specific compensation survey date for adjustment of key employees' wage in accordance with the Cooperative's Board approved compensation policy. Other officers wage is determined by the CEO in accordance with Cooperative guidelines in conjunction with industry survey information.
