990, PART I, LINE 1, SIGNIFICANT ACTIVITIES The following awards highlight a few of the many sponsored projects that Health Research, Inc has administered funding for during the reporting period: Data Analysis and Collaboration to Maximize Overdose Prevention The NYSDOH/Health Research Inc. seeks to increase Prescription Monitoring Program (PDMP) use among practitioners and collaborate with local health departments (LHDs) and State partners to incorporate PDMP data into overdose prevention planning. This project has two objectives. (1) NYSDOH, Bureau of Narcotic Enforcement (BNE) will collect qualitative data from practitioners to identify barriers to PDMP searching, distribute unsolicited reports to practitioners, and develop and disseminate provider education to increase PDMP utilization. (2) NYSDOH/BNE will partner with the Office of Drug User Health (ODUH) and New York State Association of County Health Officials (NYSACHO) to facilitate relationship building with LHDs. MRC State, Territory and Tribal Nations Representative Organizations for Next Generation MRCSTRONG The goals of this project are: 1) to enhance the state's existing Emergency System for Advance Registration of Volunteer Health Professionals (ESAR-VHP) capabilities; 2) assist NYS MRC Units in rebuilding their organizations from the foundation up. The two (2) principal objectives: 1) to use lessons learned from the NYS response to COVID-19 to develop the ServNY ESAR-VHP into a comprehensive Volunteer Management System (VMS) platform; 2) to provide the thirty (30) existing NYSMRC units with the necessary tools and associated funding required to provide for centralized background checks, uniform and badging, spontaneous volunteer capacity, and promotional campaigns for long-term MRC grown and sustainability. BOLD NYSDOH Coalition Addressing Risk Reduction, Early Detection, and ADRD Diagnosis BOLD project focuses on 3 priority initiatives intended to expand the NYSDOH Alzheimer's Disease and Related Dementias (ADRD) Coalition, promote brain health and ADRD risk reduction strategies and enhance the availability of detection and diagnostic services in NYS. Project initiatives include: 1. Expanding the ADRD Coalition to include representation on the NYS Coordinating Council for Services Related to ADRD (Council) by the NYS Chronic Disease. Opportunities will be explored to integrate brain health and ADRD risk reduction strategies into relevant chronic disease public awareness messaging, 2. Enhancing the public health approach to ADRD in NYS by leveraging current relationships between the NYSDOH ADP, area Offices of Aging, the Alzheimer's Association and local public health departments to build resources that promote brain health and risk reduction strategies to consumers at a local level, and 3. Increasing access to early detection and diagnostic services for those individuals suspected of having ADRD. Characterization of novel kin recognition system in mycobacteria Mycobacterial diseases are a huge burden on global public health. There is a need to better understand the biology of mycobacteria. This project aims to characterize a novel mechanism of chromosomal DNA transfer that has been described in both fast- and slow growing mycobacteria and, as hypothesized, has played a major role in mycobacterial cell-cell communication and the evolution of mycobacterial pathogens.
990, Part III, Line 4a. Federally Funded HIV AIDS Prevention and Care The New York State Department of Health AIDS Institute established the Office of Uninsured Care Programs (the Programs) to provide access to medications and medical services for uninsured or underinsured New York State residents living with or at risk of acquiring HIV who meet established eligibility criteria. The AIDS Drug Assistance Program (ADAP) began in 1987 as part of a national program to provide free HIV/AIDS drugs to low-income individuals not covered by Medicaid or adequate third-party insurance. New York State expanded the program to include ambulatory care and home care services, insurance continuation and pre-exposure prophylaxis. The programs are funded through partnerships between the State and federal governments and between the State and the New York City, Long Island and Lower Hudson Ryan White Part A regions. Both federal and state statute/regulation governs the programs. The federal Ryan White HIV/AIDS Treatment Extension Act of 2009 includes statutory authority for AIDS Drug Assistance Programs throughout the nation, and associated appropriations provide federal funding to support the programs. In addition, State regulation in NYCRR, Title 10, Sub-part 43-2 governs the application and eligibility determination process and establishes the rights and responsibilities of applicants, participants, and providers. The Programs employ a dual approach to carry out their mission. First, the Programs empower the individual to seek and access care by allowing the individual to choose a provider and receive care/drugs without cost. Second, the Programs supply a stable and timely funding stream to health care providers, enabling them to use the revenues to develop program capacity to meet the needs of uninsured and underinsured people. The Programs are a key component in New York State's response to the HIV/AIDS epidemic. The Programs provide rapid access to antiretroviral (ARV) therapy and ensure universal access to care that is essential to improve health outcomes, achieve viral suppression, and reduce the risk of transmission. The Programs are comprised of five components: ADAP (AIDS Drug Assistance Program) provides free medications for the treatment of HIV/AIDS and opportunistic infections. ADAP Plus provides free primary care services at selected clinics, hospital outpatient departments, office-based physicians, and laboratory vendors. HIV Home Care Program provides coverage for home care services to chronically medically dependent individuals as ordered by their physician. ADAP Plus Insurance Continuation (APIC) can pay health insurance premiums for ADAP-eligible clients. PrEP-AP to prevent HIV infection by providing access to basic primary care services to support HIV-negative individuals at risk of acquiring HIV. The Programs can serve as a transition to Medicaid by providing interim assistance to persons eligible for but not yet enrolled in Medicaid or assist in meeting spend-down requirements. Individuals with health insurance who need assistance with meeting their deductibles or co-payments are also eligible. The Programs will coordinate benefits with their insurance company. Ryan White HIV/AIDS Program (RWHAP) legislation defers to individual states to define what qualifies as low-income eligibility and any additional program restrictions. New York State has codified its responsibilities under the Department of Health Administrative Rules and Regulations State Aid and Funding subchapter (10 CRR-NY 43-2) most recently updated in 2019. The UCP enrollment process has been revised to facilitate same-day enrollment for all participating providers, as well as applicants who apply on their own. The addition of this expansion decreases the program approval time for participants allowing for expedited ART, in accordance with the AIDS Institute policy on Rapid Initiation of Antiretroviral Therapy, which identifies every new diagnosis as an immediate call to action for every provider, with the goal of rapid initiation of treatment and the gold standard of same-day initiation of treatment. This policy supports the "U=U" message that individuals with a sustained undetectable viral load will not sexually transmit HIV. New York State's Uninsured Care Programs are the most comprehensive in the nation, offering a full scope of services to persons with HIV/AIDS. The UCP serves over 24,000 uninsured and underinsured persons living with HIV/AIDS annually. The Programs serve all populations affected by HIV/AIDS in New York State. The Programs served 24,649 participants during the 2023/2024 fiscal year. Cumulative statewide ADAP enrollment as of 4/1/2024 was 121,007, with 18,099 currently active. ADAP Plus has a cumulative enrollment of 109,124 participants with 17,301 active as of 4/1/2024. The HIV Home Care Program had a cumulative enrollment of 5822 participants with an active caseload of 24. ADAP Plus Insurance Continuation (APIC) had a cumulative enrollment of 18,612 participants as of 4/1/2024 with 4523 active. The majority of ADAP participants are persons of color: 34.1 percent are Black; 41.6 percent are Hispanic; 20 percent are White; three percent are Asian/Pacific Islander/Native American. The UCP/ADAP has developed a program-specific cascade of care which demonstrates that the programs have been successful in linking uninsured and underinsured persons to continuous care and achieving viral suppression. As a result of these activities, 98% of program participants who were active in the program for at least one year show evidence of care and 95% are virally suppressed.

PUBLIC HEALTH EMERGENCY AND HOSPITAL PREPAREDNESS The Centers for Disease Control and Prevention (CDC) Public Health Emergency Preparedness (PHEP), the Administration for Strategic Preparedness and Response (ASPR) Hospital Preparedness Program (HPP), and ASPR Medical Reserve Corps State, Territory and Tribal Nations, Representative Organizations for Next Generation (MRC-STRONG) funding supports the efforts of the New York State Department of Health (NYSDOH) Office of Health Emergency Preparedness (OHEP). NYSDOH OHEP continues to work with the State Office of Emergency Management (State OEM) and other State Agencies, Local Health Departments (LHD) and private and public-sector partners to build a solid emergency preparedness and response foundation to respond to any crisis or emergent situation in the State. NYSDOH OHEP operates an integrated and comprehensive health emergency preparedness program, and these efforts can be categorized across several areas. 1. NYSDOH OHEP continues to sustain its fully developed Incident Management System (IMS). Accomplishments from the period 04/01/2023 to 03/31/2024 NYSDOH Informatics finished initial proof of concept technology review and made development plans for the switch of IMS Dashboard to Tableau technology. NYSDOH Informatics obtained licenses and educational material for Tableau and staff have been learning the technology. NYSDOH Division of Epidemiology (DOE) cross trained staff for Emergency Support Function (ESF) and Emergency Operations Center (EOC) representation which enhanced overall incident management capability. 2. NYSDOH OHEP continues to support and provide updated guidance for distribution and dispensing of Medical Countermeasures (MCM), including vaccines and medications, using local Points of Dispensing (POD) and Clinical Operations (ClinOps). Accomplishments from the period 04/01/2023 to 03/31/2024 NYSDOH Informatics continued meetings with the Countermeasure Data Management System (CDMS) workgroup to determine next steps to enhance the system while updating middle layer technology stack along with security scan improvements. NYSDOH OHEP conducted quarterly Cities Readiness Initiative (CRI) conference calls with the 14 CRI jurisdictions. 3. NYSDOH continues to support the Medical Emergency Response Inventory Tracking System (MERITS). MERITS is an electronic inventory management system that supports the Strategic National Stockpile (SNS) and the State's Medical Emergency Response Cache (MERC) warehouse operations. This includes processing orders, receiving, shipping, reporting, and maintaining a master inventory of all assets. NYSDOH OHEP continues to sustain medical materiel management and distribution capacity to respond to a public health threat with MCM and/or Durable Medical Equipment (DME) to provide for a timely and effective response. Accomplishments from the period 04/01/2023 to 03/31/2024 NYSDOH Informatics completed updates to the MERITS application and released the new version following training of NYSDOH OHEP Logistics staff. Preventative maintenance of DME continued, including ventilators, oxygen concentrators and pulse oximeters. Expired resources were identified across all State medical warehouses for disposition. NYSDOH OHEP in collaboration with the Health Care Coalition (HCC) Readiness and Response Coordinator completed update of Critical Asset Survey (CAS) questions to collect information about integrity of supply chain, Personal Protective Equipment (PPE) and other durable medical goods. 4. NYSDOH continues to support informatics infrastructure to provide electronic, secure systems and applications for emergency preparedness, response, and recovery. NYSDOH will sustain and improve systems for information exchange. In-place architecture was leveraged, and new components were added that link NYSDOH with its emergency preparedness and response partners and promote the exchange of data with stakeholders, bi-directionally, while ensuring appropriate privacy protection. Accomplishments from the period 04/01/2023 to 03/31/2024 Based on lessons learned from the COVID-19 response, Critical Specimen Transport (CST) training was updated to align with new enhancements to the operation, including a smart phone-based application to support transport communication and documentation. 78 staff were trained in basic CST operations using recorded webinar trainings. The overall percentage of emergency risk communication on NYSDOH social media channels (Facebook, Twitter, Instagram, etc.) was approximately 60% of overall posts during this period and covered storms, extreme heat, flooding, and air quality warnings. All four (4) Health Emergency Preparedness Coalitions (HEPCs) conducted two (2) Interoperable Communication (IOC) drills. NYSDOH Informatics updated the Health Commerce System (HCS) and Communications Directory technology to ensure security and reliability to include password policies, multi-factor authentication for specific applications, and movement to cloud based technologies. 5. NYSDOH OHEP builds public health emergency response capacity through extensive training offerings and close collaboration with LHDs, hospitals, and other partners. These trainings allow NYSDOH OHEP to enhance readiness and ensure that partners can receive any needed Technical Assistance (TA). In addition, NYSDOH OHEP conducts regular exercises to test different aspects of public health emergency response. Accomplishments from the period 04/01/2023 to 03/31/2024 NYSDOH Informatics provided monthly HCS, Health Electronic Response Data System (HERDS) and Integrated Health Alerting Network System (IHANS) trainings to State, regional and local partners. 162 were trained in Introduction to HCS, 133 were trained in HERDS data entry, 132 were trained in the HCS Communications Directory, 71 received IHANS certification and 457 were trained in Evacuation of Facilities in Disasters System (eFINDS). NYSDOH OHEP participated in an ESF-6 training and exercise on May 2, 2023, that included discussion about Disaster Recovery Centers (DRC). The recorded training was shared with ESF-6 partners. NYSDOH OHEP provided a "Working Effectively with Tribal Nations During Public Health Emergencies" webinar on May 12, 2023. Fifty-five partners participated in the webinar. Additionally, OHEP sent four (4) communications to Tribal Nations to support public health preparedness and response planning. NYSDOH DOE cross trained staff for ESF and EOC Representation which enhanced overall incident management capability. Annual Healthcare Facility Evacuation Center (HEC) exercise held August 29th, 2023. Over 50 individuals, between players, observers, and simulation cell staff participated in the exercise. This was inclusive of many internal NYSDOH partners and external partners and resources. NYSDOH OHEP Volunteer program held training on "Writing a Volunteer Management Plan." The course resulted in 81 participants trained with representation from all 57 LHDs. NYSDOH OHEP Volunteer program trained MRC Unit leaders on conducting criminal background checks for MRC members. The class set the elements of completion for writing a criminal background check policy for their unit. This resulted in 28 of 32 MRC units participating and 47 students trained. NYSDOH distributed twelve (12) monthly Aware Prepare electronic newsletters to coalition members on upcoming trainings related to preparedness. NYSDOH converted the Psychological First Aid (PFA) training and eFINDS training to a virtual delivery method. NYSDOH OHEP participated in the annual Integrated Preparedness Planning Workshop (IPPW) on February 1, 2023, conducted by State OEM. NYSDOH OHEP reviewed, provided feedback, and approved revisions to four (4) HEPC training plans. All four (4) HEPCs completed the Medical Response and Surge Exercise (MRSE) to determine surge capacity among acute care beds and assess strengths and areas for improvement. All four (4) HEPCs submitted an After-Action Report/Improvement Plan (AAR/IP). 6. NYSDOH continues to sustain and build public health laboratory testing capability of the NYSDOH Wadsworth Center (WC), to include testing of clinical, environmental, food and water samples. Laboratory testing continues to be a priority area including coordination and communication efforts and building additional capabilities. Laboratory Response Network (LRN)-Biological (B) goals include rapid testing using LRN protocols and reporting to submitters to provide information for prompt decision making. LRN-B will sustain the ability for contacting the sentinel laboratories in NYS through the HCS. LRN-Chemical (C) goals include the adoption of technology to process and maintain the positive identification of the large number of specimens that are handled during surge testing and to use the reference materials that are now supplied by the CDC for method improvement and the full validation of analytica
990, Part III, Line 4b. PUBLIC HEALTH AND CARE EMERGENCY PREPAREDNESS PREPARDNESS CONTD. 6. The NYSDOH continues to sustain and enhance its communicable disease surveillance systems that collect information from multiple data streams daily to ensure a baseline of epidemiologic data for NYS. NYSDOH continues to sustain the capability to analyze and interpret epidemiologic data that is critical to the practice of public health. Accomplishments from the period 04/01/2022 to 03/31/2023 NYSDOH DOE, WC and NYSDOH Regional Emergency Preparedness staff reviewed past Biological Disease Surveillance (BDS) planning, local plan examples and assessed future BDS planning within context of overall Inhalation Anthrax planning. The NYSDOH DOE continued ongoing case investigation, contact tracing and community mitigation efforts. Laboratory data from Electronic Clinical Laboratory Reporting System (ECLRS) and case information from Communicable Disease Electronic Surveillance System (CDESS) continued to be used extensively during all public health response efforts. 100% of NYS Emergency Department (ED) sites are submitting data to ESSENCE. Investigations were initiated for 99.7% of Category I disease reports within 24 hours. Investigations were initiated for 97.8% of Category II disease reports within three (3) days. Investigations were initiated for 98.5% of Category III disease reports within five (5) days. 98% of Category A investigations were completed. 90.6% of Category B investigations were completed. 88.0% of Category C investigations were completed. ECLRS data has been used for many public facing dashboards related to the COVID pandemic response and NYSDOH added a new dashboard displaying syndromic surveillance data, a first for NYSDOH. No new syndromes were added for surveillance, but the data continued to be heavily used for public health response efforts (e.g., drug overdose surveillance, polio, heat/smoke, etc.). Program Period Successes: COVID-19 Response NYSDOH continued to generate inventory reports using MERITS to quickly assess stockpiled supply of PPE and DME in the MERC warehouses and continued to monitor inventory, maintain DME and replenish supplies when needed. NYSDOH continued to follow established recovery procedures and capture lessons learned and best practices from the COVID-19 pandemic response. NYSDOH continued to coordinate with emergency response partners including Division of Homeland Security and Emergency Services (DHSES), State OEM and the Office of General Services (OGS) demobilizing, determining storage and maintenance and/or redistributing assets to improve readiness for any possible future COVID-19 or other infectious disease outbreaks. NYSDOH Informatics continued to support and maintain HERDS with weekday COVID-19 surveys for Hospitals, Nursing Homes (NH) and Adult Care Facilities (ACF) to report on and track facility operating status, staffing levels, bed availability and resource inventory. NYSDOH Public Affairs Group (PAG) continued to work with the Governor's Office to provide New Yorkers with comprehensive messaging order to reduce the impact of COVID-19. The primary effort was to maintain the COVID-19 public website and to share informational resources with LHDs and health care organizations. Public outreach campaigns included messaging to classify booster doses versus standard doses for those that previously did not receive the vaccine in any form. This proved to be a confusing topic for the public and PAG maintained an extensive and continually updated Frequently Asked Questions (FAQs) page on the public website. NYSDOH continued efforts to ensure all populations understood the status of the pandemic, including distribution of CDC guidance for people with disabilities and those who use American Sign Language (ASL) and braille. To help local jurisdictions and the public to understand the impact of the pandemic, data summaries and dashboards were posted on public websites to show the number of COVID-19 deaths compared to other leading causes of death in NYS. Vaccine efforts continued to be focused on LHD and provider-level operations, with continuing "office hours" for provider questions and input. Significant efforts were conducted to provide guidance on and updates to the CDMS application that is used by LHDs to track administration of COVID-19 vaccinations. Wadsworth Center continued performing SARS-CoV-2 sequencing to monitor currently circulating variants. WC secured a courier to provide transport of urgent clinical specimens to WC and wastewater samples to consortium laboratories for sequencing to ensure that NYSDOH continues to monitor SARS-CoV-2 variants throughout NYS.
Form 990, Part III, Line 4c, CANCER RESEARCH Roswell Park Cancer Institute is a multidisciplinary, translational comprehensive cancer center, with a mission to eliminate cancer's grip on humanity by unlocking its secrets through personalized approaches and unleashing the healing power of hope. RPCI utilizes its comprehensive cancer support grant (CCSG) through HRI to build on unique strengths and opportunities, moving laboratory science into advanced treatment, prevention, diagnosis, and education. Roswell Park Cancer Institute's revolutionary research model of a "multidisciplinary approach" to cancer with scientists and clinicians working in concert and in consult has become the standard by which all modern-day comprehensive cancer centers are measured. HRI continues to work with RPCI leadership in all of their research endeavors. HRI's cancer research grant portfolio supports a nationally recognized research enterprise at RPCI. The Buffalo comprehensive cancer center is one of only 56 comprehensive cancer centers in the country. Health Research Incorporated (HRI) provides effective pre-and-post-award evaluation, solicitation, and administration of external research support from Federal and State agencies, not-for-profit foundations, and commercial firms in accordance with the requirements of the sponsor. The HRI/Grants Management staff (20 FTES) support more than 651 active research related projects annually for RPCI, providing timely and detailed revenue and expenditure information for internal and external review, as well as analysis with Institute investigators. HRI's superior Grant Administration expertise and its ability to attract highly qualified research staff have contributed to the continued upward trajectory of the institution and its cancer research. Form 990, Part VI, Section B, line 11b The Annual Form 990 is initiated by the Corporate Controller and developed in conjunction with the Assistant Controller and the Executive Director. The final draft is reviewed by the three key personnel listed above. A comparative analysis to the prior year's filing is conducted and then the final draft is presented to Health Research, Inc's independent audit firm for validation. Once finalized by the independent audit firm, the Executive Director presents and reviews the Form 990 filing with the Corporation's Secretary/Treasurer and if requested, the Corporation's Vice President. A copy of the 990 is provided to all Directors of the HRI Board for their review prior to filing.
Form 990, Part VI, Section B, line 12c HRI Conflict of Interest Policies are reviewed annually and if appropriate, revised by the Corporation's Counsel and Executive Director. The most recent version of the HRI Employee Conflict of Interest Policy was updated in January 2018. The policy is provided to employees on an annual basis via HRI's online training system. Individuals are required to attest to reading the policy; confirmation of which is electronically maintained by Corporate Human Resource Staff. In addition to the annual certification, all new employees are provided with a copy of Employee Conflict of Interest Policy in conjunction with New Hire Orientation, and the policy is available on the HRI website. All potential conflicts must be disclosed in writing to the Corporate Office for review and consideration by management.
