COMMUNITY HOSPITAL OF ANACONDA IS COMPRISED OF A 25-BED CRITICAL ACCESS HOSPITAL AND A 62-BED NURSING HOME FACILITY WHICH PROVIDES HEALTH CARE SERVICES. THE HOSPITAL PROVIDED 3,984 DAYS OF INPATIENT ACUTE CARE SERVICES, 194,800 OUTPATIENT VISITS, 10,175 EMERGENCY ROOM/CONVENIENT CARE VISITS, AND 2,368 SURGERIES. THE NURSING HOME PROVIDED 7,001 DAYS OF RESIDENT SERVICES FOR THE YEAR ENDED DECEMBER 31, 2023. THE HOSPITAL PROVIDES CARE TO PERSONS COVERED BY GOVERNMENTAL PROGRAMS AT OR BELOW COST AND TO INDIVIDUALS WHO ARE UNABLE TO PAY. THE UNREIMBURSED CHARGES OF PROVIDING CARE TO THESE PATIENTS WAS 454,359 FOR CHARITY CARE, 44,731,106 FOR MEDICARE, 10,062,487 FOR MEDICAID, AND 20,348,401 FOR OTHER THIRD PARTY PAYORS FOR THE YEAR ENDED DECEMBER 31, 2023.

SCHEDULE H, PART VI, LINE 2 COMMUNITY HOSPITAL OF ANACONDA COMPLETED A COMMUNITY NEEDS ASSESSMENT IN THE SUMMER OF 2022. THE LAST ASSESSMENT WAS COMPLETED IN 2019. THE ASSESSMENT WAS COMPLETED WITH ASSISTANCE FROM THE MONTANA OFFICE OF RURAL HEALTH, WITH DATA COLLECTED BY STEERING COMMITTEE MEETINGS, KEY INFORMANT INTERVIEWS, FOCUS GROUPS AND A SURVEY MAILED TO 800 RANDOM HOUSEHOLDS IN OUR SERVICE AREA. THE SURVEY RESULTED IN A 16% RESPONSE RATE, WHICH IS DEEMED STATISTICALLY SIGNIFICANT. THE TOP THREE ISSUES/OPPORTUNITIES WERE IDENTIFIED: "IMPROVE THE AWARENESS & ACCESS TO HEALTHCARE SERVICES/RESOURCES THROUGHOUT THE SERVICE AREA; "ENHANCE ACCESS TO HEALTH EDUCATION AND HEALTHY LIFESTYLE OPPORTUNITIES THROUGHOUT THE SERVICE AREA; "STRENGTHEN LOCAL PARTNERSHIPS AND COLLABORATIONS THROUGHOUT THE SERVICE AREA TO ENHANCE THE COMMUNITY'S ACCESS TO SERVICES AND RESOURCES. THE CHA BOARD OF TRUSTEES APPROVED THE NEEDS ASSESSMENT IMPLEMENTATION PLAN AT THE FEBRUARY 2023 BOARD MEETING. THE PLAN ESTABLISHES GOALS, STRATEGIES FOR EACH GOAL, ANTICIPATED IMPACT, PLAN TO EVALUATE IMPACT AND MEASURE OF SUCCESS. ACTIVITIES AND STRATEGIES WERE DEVELOPED FOR EACH GOAL WITH A CORRESPONDING IMPLEMENTATION GRID. THE FOLLOWING GOALS AND STRATEGIES WERE APPROVED IN FEBRUARY 2023: GOAL 1: IMPROVE THE AWARENESS & ACCESS TO HEALTHCARE SERVICES/RESOURCES THROUGHOUT THE SERVICE AREA. STRATEGY 1: ENHANCE ACCESS TO LOCAL HEALTHCARE SERVICES AND RESOURCES. PROGRESS: MYCHART ACTIVATION CODES ARE NOW LOCATED WITHIN THE AFTER-VISIT SUMMARY. PHONE IMPROVEMENTS HAVE BEEN MADE FOR AUTO-TRANSFER OF CALLS WHICH DISPERSE INCOMING CALL VOLUMES MORE EQUALLY FOR BETTER COVERAGE. CARE COORDINATOR HAS IMPLEMENTED AN ANNUAL WELLNESS VISIT CAMPAIGN. PROVIDER IS AVAILABLE FOR DERMATOLOGY APPOINTMENTS. PROVIDERS IN CLINICS HAVE TRIALED EXPANDED CLINIC HOURS. CHA IS ACTIVELY RECRUITING FOR AN LCSW. STRATEGY 2: ENHANCE AWARENESS OF LOCAL HEALTHCARE SERVICES AND RESOURCES. PROGRESS: CHA LAUNCHED A NEW WEBSITE WHICH IS EASIER TO NAVIGATE FOR PATIENTS AND EASIER TO UPDATE FOR OUR STAFF. WE HAVE ADOPTED USING POSTCARDS, DIRECT MAIL CAMPAIGNS AND ADVERTISEMENTS TO RAISE AWARENESS OF AVAILABLE SERVICES AND RESOURCES. WE ALSO CONTINUE TO ENGAGE WITH LOCAL COMMUNITY GROUPS SUCH AS HOSA, ASD, ANACONDA FAMILY RESOURCE CENTER AND MORE TO SHARE AND DISPERSE A CONSISTENT MESSAGE. GOAL 2: ENHANCE ACCESS TO HEALTH EDUCATION AND HEALTHY LIFESTYLE OPPORTUNITIES THROUGHOUT THE SERVICE AREA. STRATEGY 1: CHAMPION LOCAL HEALTH EDUCATION AND HEALTHY LIFESTYLE OPPORTUNITIES. PROGRESS: CHA HAS RECRUITED A FULL-TIME OBSTETRICS MANAGER TO BOOST COMMUNITY ENGAGEMENT. WE CONTINUE TO OFFER SUCCESSFUL DIABETES PREVENTION PROGRAMS FOR THE COMMUNITY. CHA'S REHAB DEPARTMENT HAS LAUNCHED A JOINT REPLACEMENT CLASS FOR PATIENTS HAVING TOTAL KNEE AND HIP REPLACEMENTS. WE ALSO PROMOTE AND SPONSOR COMMUNITY RECREATIONAL ACTIVITIES AND OFFER SPORTS PHYSICALS AT A DISCOUNTED PRICE FOR LOCAL ATHLETES. GOAL 3: STRENGTHEN LOCAL PARTNERSHIPS AND COLLABORATIONS THROUGHOUT THE SERVICE AREA TO ENHANCE THE COMMUNITY'S ACCESS TO SERVICES AND RESOURCES. STRATEGY 1: CHAMPION LOCAL INITIATIVES THAT ASSESS GAPS AND ALIGN RESOURCES. PROGRESS: CHA WILL CONTINUE TO PARTICIPATE IN COMMUNITY AND COUNTY LEVEL CONVERSATIONS TO DETERMINE HOW WE CAN PARTNER TOGETHER. COMMUNITY HOSPITAL OF ANACONDA (CHA) DISSEMINATED THE COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) AND IMPLEMENTATION PLAN BY POSTING BOTH DOCUMENTS CONSPICUOUSLY ON CHA'S WEBSITE (WWW.COMMUNITYHOSPITALOFANACONDA.ORG) AS WELL AS HAVING COPIES AVAILABLE AT THE FACILITY SHOULD COMMUNITY MEMBERS REQUEST TO VIEW THE COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) OR THE IMPLEMENTATION PLANNING DOCUMENTS. THE STEERING COMMITTEE, WHICH WAS FORMED SPECIFICALLY AS A RESULT OF THE CHNA PROCESS IN ORDER TO INTRODUCE THE COMMUNITY TO THE ASSESSMENT PROCESS, WAS INFORMED OF THE IMPLEMENTATION PLAN TO SEE THE VALUE OF THEIR INPUT AND TIME IN THE CHNA PROCESS, AS WELL AS HOW COMMUNITY HOSPITAL OF ANACONDA IS UTILIZING THEIR INPUT. THE STEERING COMMITTEE, AS WELL AS THE BOARD OF DIRECTORS, WILL BE ENCOURAGED TO ACT AS ADVOCATES IN DEER LODGE COUNTY AS THE FACILITY SEEKS TO ADDRESS THE HEALTH CARE NEEDS OF THEIR COMMUNITY. CHA EMPLOYS OVER 400 INDIVIDUALS, WITH AN ANNUAL PAYROLL IN EXCESS OF 47 MILLION. THE REMAINING ORIGINAL MEMBERS MAY ATTEND THE MEETING; HOWEVER, THEY ARE NOT ACTIVE MEMBERS AND DO NOT HAVE VOTING RIGHTS.

IN REGARD TO SETTING THE SALARY LEVELS OF PHYSICIANS, CHA UTILIZES THE MGMA PHYSICIAN COMPENSATION AND PRODUCTION SURVEY.
