The final form 990 was then changed accordingly and signed by the Executive Director before filing electronically.
Form 990, Part VI, Section B, Line 12c	Persons covered by the conflict of interest policy will annually disclose or update the Chairman of the Board of Directors on a form provided by the organization of their interests that could give rise to conflicts of interest, such as a list of family members who have substantial business or investment holdings and other transactions or affiliations with businesses or other organizations of those family members. The Chairman of the Board will address each interest disclosed and act according to the guidelines of the conflict of interest policy.
