LEHIGH VALLEY HOSPITAL - DICKSON CITY (LVH-DC) A NEW STATE-OF-THE-ART HOSPITAL CAMPUS LOCATED IN DICKSON CITY, PENNSYLVANIA.WITH OVER 400 PHYSICIANS WITH CLINICAL PRIVILEGES AT LVH-POCONO AND LVH-DICKSON CITY AND OVER 1,800 STAFF MEMBERS, OUR ORGANIZATION'S MISSION IS TO PROVIDE WORLD-CLASS HEALTHCARE TO OUR COMMUNITY FOR THOSE SERVICES MOST CRITICAL TO THE COMMUNITY'S HEALTH NEEDS. AFTER BECOMING A PART OF THE LEHIGH VALLEY CANCER INSTITUTE IN 2017, MEMORIAL SLOAN KETTERING (MSK) CANCER ALLIANCE MEMBERSHIP WAS EXTENDED TO LVH-POCONO AT THE DALE AND FRANCES HUGHES CANCER CENTER IN 2018. ONCOLOGY TEAMS REVIEW PROCESSES AND CLINICAL STANDARDS ACROSS SIX DISEASE SPECIALTIES AND DISCIPLINES WITHIN THE CANCER INSTITUTE - MELANOMA, BREAST, COLON, ENDOMETRIAL, LUNG AND PROSTATE - TO ENSURE ALIGNMENT WITH MSK STANDARD OF CARE. SINCE THAT TIME, WE HAVE INITIATED SITE-SPECIFIC DISEASE MANAGEMENT TEAMS WHO SPECIALIZE IN A PARTICULAR TYPE OF CANCER CARE. THEY IDENTIFIED MULTIPLE WAYS THAT THE COVID-19 PANDEMIC HAS NEGATIVELY IMPACTED THE COMMUNITY AT ALL AGE LEVELS. FOR CHILDREN IN SCHOOL, PARTICIPANTS IDENTIFIED HIGHER RATES OF DEPRESSION AND ANXIETY AMONG STUDENTS. AMONG OLDER ADULTS, THERE HAS BEEN AN INCREASE IN SOCIAL ISOLATION AND MENTAL HEALTH CONCERNS. FOR ALL SEGMENTS OF THE POPULATION, THE PANDEMIC HAS RESULTED IN INCREASED STRESS. ADDITIONALLY, AMONG PARTICIPANTS IN MANY OF THE COMMUNITY MEETINGS AND INTERVIEWS, ALCOHOL USE WAS REPORTED TO BE HIGH AND RISING WITHIN THE COMMUNITY. THIS INCREASE IS BEING SEEN IN A RANGE OF AGES, RACES, GENDERS, AND SOCIOECONOMIC GROUPS IN MONROE COUNTY. IN TERMS OF DRUG AND ALCOHOL SERVICES, PARTICIPANTS IDENTIFIED A NEED FOR MORE HALF-WAY HOUSES, INPATIENT PROGRAMS, AND LONGER-TERM TREATMENT FACILITIES AS WELL AS DETOX AND OTHER ALTERNATIVES TO HELP INDIVIDUALS BEFORE THEY MAY NEED INPATIENT TREATMENT. DATA FROM THE COUNTY HEALTH RANKINGS CORROBORATE THIS CONCERN, SINCE 19.7% OF MONROE COUNTY RESIDENTS SELF-REPORT EXCESSIVE DRINKING. ABBY LECHTER, A FAMILY MEDICINE PHYSICIAN BASED AT LVH-CEDAR CREST, EDUCATED ABOUT A DOZEN PRIMARY CARE INTERNISTS AT LVH-POCONO ABOUT SUBSTANCE USE DISORDERS AND STATE GUIDELINES FOR MAKING NALOXONE AVAILABLE.- NURSES ON MED/SURG UNITS WHO SEE PATIENTS WITH BEHAVIORAL HEALTH CONCERNS HAVE BEEN TRAINED. EXPANDING SERVICES TO MEET THE BEHAVIORAL HEALTH NEEDS OF THE COMMUNITY:- A NEW PSYCHIATRIST, DR. ALI, WAS HIRED IN FY23 TO START IN FY24.- LVH-POCONO CONTINUED TO USE LOCUM TENENS FOR PSYCHIATRY AND PSYCHOLOGY WHILE RECRUITING PERMANENT STAFF.- LVH-POCONO ESTABLISHED A NEW PROCESS WITH THE COUNTIES TO PROVIDE A WARM HAND-OFF FOR ANYONE WHO IS ADMITTED WITH A SUICIDE THREAT. THE WARM HAND-OFF INCLUDES A REFERRAL FROM THE COUNTY FOR A CASE MANAGER WHO FOLLOWS UP WITH THE PATIENT FOR SIX MONTHS.- OUR NURSE FAMILY PARTNERSHIP (NFP) PROGRAM RECEIVED A THREE-YEAR GRANT OF $899,000 PER YEAR PLUS $86,600 FROM CHILDREN AND YOUTH SERVICES. WE ARE WORKING TO ADDRESS TRAUMA AND TRACK DEVELOPMENTAL MILESTONES FROM PREGNANCY THROUGH TWO YEARS OF AGE. WE CAN ALSO PROVIDE VERY EARLY REFERRALS TO EARLY INTERVENTION SERVICES. THE NFP'S BREASTFEEDING PROGRAM HAS RESULTED IN DRAMATICALLY INCREASED BREASTFEEDING FROM 3 TO 6 MONTHS AND HAS ALSO SEEN INCREASES AT OTHER AGE POINTS.PRIORITY AREA: DIABETES PREVENTION (MONROE COUNTY) A GENERAL LACK OF HEALTHY HABITS AND SPECIFICALLY THE PREVALENCE OF OBESITY WERE IDENTIFIED IN MANY COMMUNITY MEETINGS AND INTERVIEWS IN MONROE COUNTY. PARTICIPANTS POINTED TO A LACK OF PHYSICAL ACTIVITY AND OUTDOOR ACTIVITIES AND UNHEALTHY FOOD CHOICES, SUCH AS PROCESSED FOOD AND FAST FOOD, AS REASONS FOR INCREASED OBESITY RATES AND OTHER HEALTH-RELATED ISSUES. THOSE WHO WERE ENGAGING IN PHYSICAL ACTIVITY REGULARLY FELT THAT IT WAS HELPING THEM TO STAY HEALTHY. FOR FAMILIES AND COMMUNITY MEMBERS WHO NEED TO ACCESS FOOD THROUGH FOOD PANTRIES, PARTICIPANTS NOTED THAT THE OPTIONS AVAILABLE THERE ARE OFTEN NOT HEALTHY, WHICH ADDS TO OBESITY LEVELS AND UNHEALTHY LIFESTYLES. MANY COMMUNITY MEMBERS SHARED THAT THEY KNEW SOMEONE WHO HAS DIABETES, AND THEY IDENTIFIED FOOD HABITS AND CHOICES AS PART OF THE CAUSE FOR THE HIGH RATES OF DIABETES. THEY POINTED OUT A NEED FOR PREVENTION PROGRAMS TO DECREASE THE RISK OF DEVELOPING DIABETES. IN MONROE COUNTY, 8.8% OF ADULTS HAVE DIABETES AND 26% OF THE MEDICARE POPULATION HAS DIABETES (ACCORDING TO DATA FROM THE CENTERS FOR DISEASE CONTROL AND PREVENTION AND THE CENTERS FOR MEDICARE AND MEDICAID SERVICES). TO ADDRESS COMMUNITY MEMBERS' CONCERNS ABOUT DIABETES, WE PILOTED A DIABETES PREVENTION AND HEALTHY LIVING PROGRAM IN FY23. THIS PROGRAM AIMS TO REDUCE DIABETES RISK BASED ON BODY MASS INDEX AND PREDIABETES STATUS. TO BE ELIGIBLE, PATIENTS MUST HAVE A BILLABLE DIAGNOSIS IN EACH OF THE THREE AREAS (FITNESS, NUTRITION, AND BEHAVIOR). PROFESSIONALS IN THE PROGRAM INCLUDE A NUTRITIONIST, A PHYSICAL THERAPIST, A BEHAVIORAL THERAPIST, AND MEDICAL OVERSIGHT. THE PROGRAM IS DESIGNED TO BE THREE MONTHS LONG; HOWEVER, PATIENTS ARE GIVEN FLEXIBILITY IN THE SCHEDULING THE FITNESS, NUTRITION, AND BEHAVIORAL COMPONENTS. THEREFORE, THE PROGRAM CAN TAKE UP TO SIX MONTHS. A SHORT VIDEO AND PAPER BROCHURE WERE DEVELOPED WITH FAMILY MEDICINE TO PROMOTE PROVIDER REFERRALS. DURING THE PILOT, PROVIDERS ARE POSTING RESULTS OF VISITS AND BIOMETRICS ON A SHARED DRIVE. EIGHTEEN WOMEN AND TWO MEN ARE ENROLLED. OUTCOMES FOR FY23 INCLUDE: 58% OF PARTICIPANTS IMPROVED BODY MASS INDEX (BMI); 47% IMPROVED ON FASTING GLUCOSE; 32% IMPROVED ON THE HBA1C MEASUREMENT; 32% IMPROVED ON BLOOD PRESSURE; 26% IMPROVED ON LDL; 16% IMPROVED ON HDL; AND 11% IMPROVED ON TRIGLYCERIDES. IN ADDITION TO THE DIABETES PREVENTION PROGRAM, WE ALSO PROVIDE SUPPORT FOR PATIENTS WITH GESTATIONAL DIABETES. OUR NUTRITIONIST MEETS WITH FOUR TO FIVE GESTATIONAL DIABETES PATIENTS PER WEEK. SHE ALSO MEETS WITH EACH PATIENT 1:1 EVERY SIX WEEKS. IN MAY, A ZOOM EVENT WAS PROVIDED TO THE LOCAL NAACP REGARDING HEALTHY EATING.
FACILITY REPORTING GROUP - A PART V, SECTION B, LINE 18E: COLLECTION ACTIVITIES ARE LIMITED TO HOSPITAL SENDING FOUR STATEMENTS REQUESTING PAYMENT. THE STATEMENTS INCLUDE INFORMATION ABOUT THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY, SOLICITING THE PATIENT PARTICIPATION IN THE FINANCIAL ASSISTANCE PROGRAM.
FACILITY REPORTING GROUP A, PART V, SECTION B, LINE 11 (CONTINUATION): IN ADDITION, LVHN OPENED A NEW CAMPUS IN DICKSON CITY IN MAY 2022. THIS IS THE FIRST COMMUNITY HEALTH NEEDS ASSESSMENT LVHN HAS CONDUCTED FOR LACKAWANNA COUNTY, AND IT IS A NEW GEOGRAPHIC AREA FOR HEALTH SERVICES FOR THE NETWORK. THEREFORE, THE NEEDS ASSESSMENT AND IMPLEMENTATION PLAN EFFORTS ARE FOCUSED ON DEVELOPING RELATIONSHIPS AND UNDERSTANDING WHAT SERVICE PROVIDERS AND THE COMMUNITY ARE ALREADY IMPLEMENTING TO ADDRESS THE PRIORITY AREAS IDENTIFIED. THIS DEVELOPMENTAL WORK WILL CONTINUE OVER THE NEXT TWO YEARS AS LVH-DICKSON CITY CONTINUES TO WORK TO ADDRESS THE TWO PRIORITY AREAS OUTLINED BELOW.AT THE LVH-DICKSON CITY HOSPITAL LOCATION IN DICKSON CITY, LACKAWANNA COUNTY, THE FOLLOWING PRIORITY AREAS WERE IDENTIFIED IN THE COMMUNITY HEALTH NEEDS ASSESSMENT IMPLEMENTATION PLAN FOR FY2023:PRIORITY AREA: FOOD ACCESS, PARTICULARLY FOR CHILDREN AND FAMILIES (LACKAWANNA COUNTY)AT LVH-DICKSON CITY, ONE OF THE NEEDS THAT WAS IDENTIFIED THROUGH THE DATA AND THAT COMMUNITY MEMBERS AND LEADERS VOICED CONCERN OVER WAS THE RATE OF OBESITY AMONG RESIDENTS IN LACKAWANNA COUNTY AND THE LACK OF ACCESS TO HEALTHY FOOD OPTIONS. IN LACKAWANNA COUNTY, 18% OF THE POPULATION RECEIVE SNAP BENEFITS (I.E., FOOD STAMPS), WHICH IS IN GREATER THAN THE STATE PERCENTAGE (14%). THIS PERCENTAGE HAS DOUBLED OVER TIME FROM 9.7% IN 2007 TO 18% IN 2017. IN ADDITION, 29% OF THE ADULT POPULATION IS OBESE, WHICH HAS ALSO BEEN INCREASING IN LACKAWANNA COUNTY SINCE 2004.COMMUNITY MEMBERS WHO PARTICIPATED IN COMMUNITY CONVERSATIONS AND INTERVIEWS ACKNOWLEDGED THAT THE DEVELOPMENT OF HEALTHY HABITS AROUND FOOD AND EXERCISE START DURING CHILDHOOD AND EDUCATION AND SUPPORTING HEALTHY HABITS EARLY ON IS NEEDED, GIVEN THE RISE IN CHILDHOOD OBESITY. OBESITY ALSO LEADS TO OTHER ISSUES SUCH AS HEART DISEASE AND DIABETES LATER IN LIFE. PARTICIPANTS SUGGESTED THAT MORE EDUCATION ABOUT HEALTH WOULD BE USEFUL AS WELL AS CONTINUED INVESTMENT IN TRAILS AND PARKS AS THESE ARE IMPORTANT COMMUNITY ASSETS.IN FY2023, PRIMARY CARE PHYSICIANS HAVE BEEN MEETING WITH A NEW NUTRITIONIST IN THE COMMUNITY AND USING HER SERVICES TO SUPPORT PATIENT NEEDS. LVH-DICKSON CITY ALSO PARTNERED WITH FRIENDS OF THE POOR AND WEINBERG FOUNDATION FOR THANKSGIVING. THEY DISTRIBUTED FOOD WHILE ALSO PROVIDING VACCINES. LVH-DICKSON CITY WILL ALSO BE PARTICIPATING IN THE HUNGER TASK FORCE IN SCRANTON. THE CITY OF SCRANTON IS PARTICIPATING IN THE WHITE HOUSE'S COMMITMENT TO END HUNGER AND BUILD HEALTHY COMMUNITIES. THROUGH THIS ENHANCED HUNGER TASK FORCE, THEY STRIVE TO WORK TOGETHER TO ADDRESS GAPS IN SERVICE DELIVERY AND BETTER EDUCATE OUR COMMUNITY ABOUT HUNGER AND FOOD INSECURITY.FINALLY, LVHN WILL BE PARTICIPATING IN THE ANCHOR ECONOMY INITIATIVE THROUGH THE FEDERAL RESERVE BANK OF PHILADELPHIA (FRB), WHICH EXPLORES HOW ANCHOR INSTITUTIONS SUSTAIN JOBS, DRIVE ECONOMIC GROWTH, AND SUPPORT EQUITABLE REGIONAL DEVELOPMENT IN NUMEROUS REGIONS ACROSS THE COUNTRY. ANCHOR INSTITUTIONS ARE LARGE, PUBLIC-SERVING ORGANIZATIONS, SUCH AS HOSPITALS AND COLLEGES OR UNIVERSITIES, WHICH PROVIDE JOBS AND SERVICES. FURTHERMORE, THEY SUPPORT LOCAL ECONOMIC ACTIVITY. THE INSTITUTE IN NORTHEASTERN PENNSYLVANIA WAS AWARDED ONE OF THE MULTI-PARTNER INITIATIVES.PRIORITY AREA: MENTAL HEALTH PREVENTION AND EDUCATION (LACKAWANNA COUNTY) COMMUNITY MEMBERS WHO PARTICIPATED IN COMMUNITY MEETINGS AND INTERVIEWS DISCUSSED THE NEED FOR ADDITIONAL MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES IN THE COUNTY, INCLUDING THE NEED FOR MORE MENTAL HEALTH PROVIDERS, PSYCHIATRISTS, AND SERVICES THAT TARGET MOTHERS WITH YOUNG CHILDREN. IN FY2023, LVPG HIRED A PRIMARY CARE PHYSICIAN FOCUSED ON LGBTQ+ NEEDS. THE PHYSICIAN STARTED IN CARBONDALE IN AUGUST 2023. IN ADDITION, LVH-DICKSON CITY IS COLLABORATING WITH THE WRIGHT CENTER TO SUPPORT THEIR PSYCHIATRY FELLOWS AS WELL AS IN A TRAINING THAT LVHN IS HOSTING CALLED INTEGRATED COMMUNITY THERAPY. THE FINANCIAL ASSISTANCE POLICY APPLIES TO UNINSURED AND UNDER-INSURED INDIVIDUALS WHO PARTICIPATE IN THE PROCESS TO EVALUATE THEIR ABILITY TO PAY FOR LVHN SERVICES.PATIENTS ARE IDENTIFIED BY LVHN REGISTRATION, BENEFITS AND VERIFICATION, CUSTOMER SERVICE, AND FINANCIAL COUNSELORS AS BEING IN FINANCIAL NEED. THE FINANCIAL COUNSELORS HELP PATIENTS COMPLETE THE APPLICATION FOR FINANCIAL ASSISTANCE. LVHN FOLLOWS THE FEDERAL POVERTY GUIDELINES TO EVALUATE ELIGIBILITY. PATIENTS WHOSE FAMILY INCOME FALLS BELOW 200% OF THE FEDERAL POVERTY GUIDELINE WILL HAVE THEIR ENTIRE BALANCE FORGIVEN FOR THEIR QUALIFYING SERVICES AT A PARTICIPATING LVHN PROVIDER. PATIENTS WITH A FAMILY INCOME BELOW 400% OF THE FEDERAL POVERTY GUIDELINES WILL HAVE A PORTION OF THEIR BALANCE FORGIVEN FOR QUALIFYING SERVICES AT A PARTICIPATING LVHN PROVIDER. PATIENTS ARE EVALUATED FOR NO COST OR REDUCED PREMIUM INSURANCE PLANS. THE LVHN FINANCIAL COUNSELORS WILL OFFER INFORMATION TO PATIENTS WHO ARE INTERESTED IN SEEING IF THEY QUALIFY FOR THESE PROGRAMS OFFERED BY COMMERCIAL INSURANCE COMPANIES.PATIENTS OFTEN EXPRESS FINANCIAL CONCERN OR NEED BY CONTACTING THE LVHN CUSTOMER SERVICE DEPARTMENTS. THE CUSTOMER SERVICE REPRESENTATIVES EXPLAIN THE PROGRAMS AVAILABLE; FINANCIAL ASSISTANCE AND SUPPORT IN APPLYING FOR MEDICAL ASSISTANCE OR INSURANCE THROUGH THE FEDERAL HEALTH INSURANCE EXCHANGE. IN ADDITION, LVHN ADVERTISES FINANCIAL ASSISTANCE IN THE LOCAL NEWSPAPER, ON OUR PUBLIC WEBSITE AND ON THE STATEMENTS SENT TO OUR PATIENTS.
PART VI, LINE 4: LEHIGH VALLEY HOSPITAL-POCONO (LVH-P) IS A PENNSYLVANIA NOT-FOR-PROFIT MEMBERSHIP CORPORATION EXEMPT FROM FEDERAL INCOME TAXES AS A CORPORATION DESCRIBED IN SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE.THE PRIMARY SERVICE AREA OF LVH-POCONO AND LVH-DICKSON CITY INCLUDES MONROE AND LACKAWANNA COUNTIES. U.S. CENSUS BUREAU DATA FOR THE 2020 CENSUS INDICATES THE PRIMARY SERVICE AREA POPULATION WAS APPROXIMATELY 755,709. BASED ON THE U.S. CENSUS BUREAU ACS, THE PRIMARY SERVICE AREA ESTIMATED POPULATION IN 2022 IS 761,897.DURING THE CALENDAR YEAR 2022, 88.4% OF THE DISCHARGES FROM LVH-POCONO AND LVH-DICKSON CITY WERE RESIDENTS OF THE PRIMARY SERVICE AREA. THE 2020 POPULATION OF THE SECONDARY SERVICE AREA, CARBON, AND PIKE COUNTIES WAS APPROXIMATELY 123,784. THE ESTIMATED 2022 POPULATION OF THE SECONDARY SERVICE AREA IS 126,018 (U.S. CENSUS BUREAU ACS). DURING THE CALENDAR YEAR 2022, 4.3% OF THE DISCHARGES FROM LVH-POCONO WERE RESIDENTS OF THE SECONDARY SERVICE AREA. DURING THE CALENDAR YEAR 2022, 7.2% OF THE DISCHARGES FROM LVH-POCONO WERE RESIDENTS OUTSIDE THE PRIMARY AND SECONDARY SERVICE AREAS. BASED ON PROPRIETARY DATA ESTIMATES (CLARITAS), THE PRIMARY SERVICE AREA CURRENT POPULATION PROJECTION WILL INCREASE 0.6% BY 2028.
