Our organization had undergone changes with the deaths of key members. When we became aware of non filed tax returns we attempted to have them filed. The initial tax return had been mailed to the IRS and received in the Ogden UT office on September 27th 2023. It was subsequently returned and we were notified that it had to be e-filed. We did not have access to third party e-file providers at that time. We had then e-filed the 990-T but inadvertently not included the 990-EZ. We have now put in policies and procedures to get all returns filed and prevent future occurrences. We are requesting abatement of penalties and interest. Thank you for your attention to this matter.
