IN 2021, THE FOUNDATION BECAME AWARE OF AN INVESTMENT ACCOUNT THAT WAS INADVERTENTLY OMITTED FROM THE 2019 FORM 990-PF. THE INVESTMENT ACCOUNT WAS NEW IN 2019 AND FUNDED WITH CONTRIBUTIONS OF INVESTMENT ASSETS. AN AMENDED FORM 990-PF HAS BEEN PREPARED AND FILED IN MAY 2021 CONCURRENT WITH THE 2020 FORM 990-PF FILING. THE 2019 AMENDED RETURN, HAS BEEN FILED TO CALCULATE THE REQUIRED REMAINING DISTRIBUTION. THE FOUNDATION FULLY DISTRIBUTED THE 2019 AMOUNT AND 2020 REQUIRED DISTRIBUTIONS BEFORE DECEMBER 31, 2021. THE AMOUNT OF REQUIRED DISTRIBUTIONS TO BE APPLIED TO 2019 IS $5327 WHICH WAS FULLY DISTRIBUTED BY DECMEBER 31, 2021 AND HAS BEEN INCLUDED AS PRIOR YEAR DISTRIBUTIONS FOR UNDISTRIBUTED INCOME.
