Form 990, Part VI, Section A, Line 4	The Association amended its Constitution during a Grand Convention held September 20, 2023. The amended Constitution was approved by the Supreme Governing Body on September 20, 2023, and the Ohio Department of Insurance on October 2, 2023.
Form 990, Part VI, Section A, Line 6	The UTUIA has approximately 36,698 policyholders who are members of the UTUIA Fraternal Benefit Society.
Form 990, Part VI, Section A, Line 7a	The members (policyholders) of the UTUIA elect delegates to the Grand Convention (held every four years) who then elect the Board of Directors.
Form 990, Part VI, Section B, Line 11b	The Form 990 is produced in the finance department under the direction of the Secretary and Treasurer. A final draft is then forwarded to the Chairperson of the Independent Governance and Audit Committee and the President of the UTUIA for review and approval before the document is filed.
Form 990, Part VI, Section B, Line 12c	The Board members are required on an annual basis to review the conflict-of-interest policy and enter their adherence and approval into the minutes.
