FINANCIAL ASSISTANCE (CHARITY CARE) AND TRAUMA CARE. PROVIDING INPATIENT AND OUTPATIENT HEALTH CARE SERVICES TO THE COMMUNITY REGARDLESS OF THE PATIENTS' ABILITY TO PAY.AS PART OF ITS COMMUNITY HEALTH STRATEGY, ADVOCATE SHERMAN HOSPITAL (ADVOCATE SHERMAN) IS COMMITTED TO PROMOTING INITIATIVES THAT ENAHANCE ACCESS TO HEALTH CARE FOR THE UNINSURED AND UNDERINSURED. AN EXAMPLE OF THIS IS THE PROVISION OF FINANCIAL ASSISTANCE. ADVOCATE SHERMAN HOSPITAL OFFERS A VERY GENEROUS FINANCIAL ASSISTANCE PROGRAM, REQUIRING NO PAYMENTS FROM THE PATIENTS MOST IN NEED, AND PROVIDING DISCOUNTS TO UNINSURED AND INSURED PATIENTS. PATIENTS EARNING UP TO SIX TIMES THE FPL, AND INSURED PATIENTS EARNING UP TO TWO AND HALF TIMES THE FPL, MAY QUALIFY FOR A FULL OR PARTIAL FINANCIAL ASSISTANCE DISCOUNT. AS AN ACCREDITED CHEST PAIN CENTER THROUGH THE SOCIETY OF CARDIOVASCULAR PATIENT CARE, ADVOCATE SHERMAN IS DEDICATED TO THE CARE OF ITS CARDIAC PATIENTS. THIS ACCREDITATION MEANS THAT THE HOSPITAL INTEGRATES THE INDUSTRY'S BEST PRACTICES TO PROVIDE THE BEST POSSIBLE OUTCOMES IN CARDIAC CARE. DATA IS ABSTRACTED, SUBMITTED AND REPORTED THROUGH THE STROKE DATA REGISTRY AND THE CENTERS FOR MEDICARE AND MEDICAID SERVICES. ADVOCATE SHERMAN IS A 255-BED NOT-FOR-PROFIT HOSPITAL LOCATED IN ELGIN, ILLINOIS, AND IS ONE OF 27 ACUTE CARE HOSPITALS IN THE ADVOCATE AURORA HEALTH SYSTEM. ADVOCATE SHERMAN PROVIDES ACUTE INPATIENT AND OUTPATIENT MEDICAL CARE TO INDIVIDUALS RESIDING IN THE GREATER ELGIN, ILLINOIS AREA. FOUNDED IN 1888, THE HOSPITAL HAS EXPANDED ITS SERVICES TO SEVERAL OFF-SITE LOCATIONS, INCLUDING THREE IMMEDIATE CARE CENTERS AND A STAND-ALONE IMAGING CENTER. AT ALL THESE SITES OF CARE, THE HOSPITAL'S 952 CREDENTIALED PHYSICIANS AND 1,681 TEAM MEMBERS (EMPLOYEES) PROVIDE QUALITY AND COMPASSIONATE CARE TO INDIVIDUALS REGARDLESS OF THEIR ABILITY TO PAY FOR SERVICES RECEIVED. THE HOSPITAL HAS OPERATED IN ITS CURRENT LOCATION ON RANDALL ROAD SINCE DECEMBER 2009. ADVOCATE SHERMAN HAS BEEN RECOGNIZED FOR ITS COMMITMENT TO CONTINUALLY IMPROVING PERFORMANCE AND PATIENT CARE AS DEMONSTRATED THROUGH ITS ACHIEVEMENT OF SEVERAL AWARDS. CONSIDERED THE NATIONAL "GOLD STANDARD" FOR NURSING EXCELLENCE AMONG HOSPITALS, THE HOSPITAL HAS RECEIVED MAGNET RECOGNITION BY THE AMERICAN NURSES CREDENTIALING CENTER, AN AFFILIATE OF THE AMERICAN NURSES ASSOCIATION. ADVOCATE SHERMAN SERVES A PRIMARY SERVICE AREA (PSA) INCLUDING 298,893 PEOPLE WHO RESIDE IN THE ILLINOIS COMMUNITIES OF ALGONQUIN, CARPENTERSVILLE, WEST DUNDEE, ELGIN, GILBERTS, HUNTLEY, LAKE IN THE HILLS AND SOUTH ELGIN.ACCORDING TO THE 2022 CHNA, IN THE ADVOCATE SHERMAN PSA, 7.8 PERCENT OF THE POPULATION IS LIVING BELOW THE FEDERAL POVERTY LELVE (FPL). THE COMMUNITIES WITH THE LARGEST PERCENTAGE OF FAMILIES LIVING BELOW THE FEDERAL POVERTY LEVEL ARE THOSE IN ELGIN (60120) AT 13.1 PERCENT, CARPENTERSVILLE (60110) AT 11.6 PERCENT AND ELGIN (60123) AT 10.1 PERCENT. THE PSA MEDIAN HOUSEHOLD INCOME IS $90,927. COMMITMENT TO THE COMMUNITY. EVEN IN THE FACE OF LOW REIMBURSEMENTS, ADVOCATE SHERMAN IS DEDICATED TO MAINTAINING A STRONG PRESENCE WITHIN ITS COMMUNITY AND CONTINUES TO MONITOR EXPENDITURES TO MAKE CERTAIN THAT THE PROGRAMS AND SERVICES SUPPORTED ARE IN DIRECT RESPONSE TO COMMUNITY NEED. THE FOCUS AREAS ARE: 1) ACCESS TO PRIMARY MEDICAL HOMES; 2) ACCESS TO BEHAVIORAL HEALTH SERVICES; 3) WORKFORCE DEVELOPMENT; 4) COMMUNITY SAFETY; 5) AFFORDABLE HOUSING; AND 6) WORKFORCE DEVELOPMENT. EACH STRATEGY FOCUS AREA AND EXAMPLES OF ADVOCATE SHERMAN HOSPITAL PROGRAMS AND ACTIVITIES ADDRESSING THAT STRATEGY ARE PROVIDED BELOW.1. ACCESS/PRIMARY MEDICAL HOMES. MOBILE INTEGRATED HEALTH (MIH). TO IMPROVE THE CARE COORDINATION AND REDUCE READMISSIONS OF THE CHRONICALLY ILL PATIENTS AT ADVOCATE SHERMAN AS THEY TRANSITION FROM HOSPITAL TO HOME, A HOSPITAL-BASED MIH SYSTEM HAS BEEN CREATED. NATIONAL DATA SHOWS THAT MOBILE CARE HELPS PREVENT HOSPITAL READMISSIONS, ESPECIALLY IN AREAS WHERE PEOPLE HAVE LIMITED ACCESS TO HEALTH CARE AND TRADITIONAL HOME HEALTH SERVICES ARE RESTRICTED DUE TO A VARIETY OF REASONS. THE HOSPITAL DONATED 335 POUNDS OF FRESH PRODUCE FOR STUDENTS WHO ARE FOOD INSECURE AT THE ELGIN COMMUNITY COLLEGE STUDENT SPARTAN FOOD PANTRY AND TO THE FOOD FOR GREATER ELGIN FOOD PANTRY. ADVOCATE SHERMAN CONTINUES TO PARTNER WITH THE UNIVERSITY OF ILLINOIS EXTENSION MASTER GARDENERS FOR THE COMMUNITY GARDEN. THEY ARE AN INTEGRAL PART OF THE PROGRAM, MAINTAINING DEMONSTRATION BEDS, HOLDING LEADERSHIP ROLES ON THE GARDEN COMMITTEE AND MENTORING MASTER GARDENER INTERNS ONSITE. BEHAVIORAL HEALTH. ADVOCATE SHERMAN SELECTED BEHAVIORAL HEALTH AS A PRIORITY, WHICH IS COMPRISED OF BOTH SUBSTANCE USE AND MENTAL HEALTH, GIVEN THAT THESE FREQUENTLY CO-OCCUR AND DUE TO THE HIGH RATES OF MENTAL HEALTH NEEDS IN THE PSA. ADVOCATE SHERMAN CONTINUES TO FOCUS ON A ROBUST PROGRAM FOR CARDIAC SERVICES INCLUDED THE COMPLETE CARE CENTER (CCC). THE CCC INCLUDES NONINVASIVE CARDIOLOGY SERVICES, HEART FAILURE RECOVERY, NEURODIAGNOSTIC, ANTICOAGULATION, HEALTH MANAGEMENT RESOURCES (HMR) WEIGHT MANAGEMENT PROGRAM, DIABETES, WOUND CARE AND HYPERBARIC OXYGEN THERAPY. THE COUNCIL SELECTED TO CONTINUE FOCUSING ON OBESITY AS A HEALTH PRIORITY, WHICH HAS A DIRECT CORRELATION TO HEART DISEASE.DIABETES. THIS INCLUDES PATIENTS AT ADVOCATE SHERMAN WHO ARE IDENTIFIED WITH ASTHMA, PNEUMONIA, COPD, OR COVID, WHO ARE REFERRED TO THE ADVOCATE SHERMAN MOBILE INTEGRATED HEALTH COMMUNITY PARAMEDICINE PROGRAM, AND THE PROGRAM HAS DEMONSTRATED A DECREASE IN EMERGENCY ROOM VISITS AND HOSPITAL READMISSIONS. ADVOCATE SHERMAN COMMUNITY HEALTH STAFF WORK COLLABORATIVELY WITH LOCAL HEALTH DEPARTMENTS, THE CANCER CARE CENTER AND AMERICAN CANCER SOCIETY TO COORDINATE LUNG CANCER SCREENINGS AND PREVENTION EFFORTS FOR RESPIRATORY CONDITIONS. ADVOCATE SHERMAN COMMUNITY HEALTH STAFF ASSIST WITH SHARING PREVENTION MESSAGES AND PRACTICES WITH PRIMARY CARE PROVIDERS IN THE PSA. HOWEVER, BECAUSE THE COUNTY HEALTH DEPARTMENTS ARE LEADING THESE EFFORTS ALREADY, AND THE NUMBER OF RESIDENTS AFFECTED IS SMALL COMPARED TO THOSE AFFECTED BY OBESITY AND BEHAVIORAL HEALTH, SEXUALLY TRANSMITTED INFECTIONS WERE NOT SELECTED AS A PRIMARY FOCUS. UNINTENTIONAL FALLS. THE CHC THOUGHT THAT UNINTENTIONAL FALLS IS MORE OF AN ISSUE WITH EDUCATION ON THE IMPORTANCE OF SAFETY, RATHER THAN A HEALTH PRIORITY OF FOCUS. THERE ARE ALREADY ORGANIZATIONS IN THE COMMUNITY WHO OFFER EDUCATION TO SENIOR POPULATIONS. THEREFORE, THE CHC DECIDED TO FOCUS ON OTHER HEALTH ISSUES. CANCER. CANCER WAS NOT INCLUDED IN THE VOTING PROCESS BECAUSE CANCER PREVENTION AND EDUCATION ARE BEING ADDRESSED THROUGH AN INTER-DISCIPLINARY TEAM INCLUDING COMMUNITY HEALTH CALLED THE INTEGRATED NETWORK CANCER PROGRAM (INCP) WHICH ADHERES TO THE COMMISSION ON CANCER STANDARDS. CHNA DATA IS PRESENTED ANNUALLY TO THE INCP LEADERSHIP AND IS USED TO DEVELOP PREVENTION, SCREENING OUTREACH AND EDUCATIONAL PROGRAMS. COVID-19. IN THE PAST YEAR, PATIENTS SEEN IN THE MIH PROGRAM HAD A 59 PERCENT DECREASE IN HOSPITALS ADMISSIONS AND A 28 PERCENT DECREASE IN EMERGENCY ROOM VISITS. ENROLLED PATIENTS HAD THE FOLLOWING HEALTH CONDITIONS: HEART FAILURE; CHRONIC OBSTRUCTIVE PULMONARY DISEASE; DIABETES; POST-MYOCARDIAL INFARCTION; PNEUMONIA AND ONE PATIENT WAS UNDERGOING CANCER CARE.COMMUNITY HEALTH WORKER. A CHW IS AN EXPERT IN LOCAL SOCIAL SUPPORT RESOURCES IN A LOCAL COMMUNITY. THE CHW OFTEN IS A MEMBER OF THE COMMUNITY HE OR SHE SERVES AND EDUCATES PATIENTS ON WHERE TO GET ASSISTANCE IN HOUSING, UTILITY ASSISTANCE OR MEDICAID ENROLLMENT. A FULL-TIME CHW IS EMBEDDED IN THE ADVOCATE SHERMAN EMERGENCY DEPARTMENT (ED). HE MEETS WITH PATIENTS AND ASSESSES THEIR SOCIAL DRIVERS OF HEALTH (SDOH) TO IDENTIFY BARRIERS AND NEEDS. HE THEN LINKS PATIENTS TO A PRIMARY CARE HOME AND OTHER AGENCIES FOR SUPPORT NEEDED. IN 2023, THE CHW WORKED WITH 302 PATIENTS AND MADE 424 REFERRALS, INCLUDING 271 REFERRALS TO PHYSICIANS, 29 REFERRALS TO ADVOCATE MEDICAL GROUP CLINICS AND 213 REFERRALS TO FQHC'S. THE TOP NEEDS OF PATIENTS SEEN WERE WOMEN'S HEALTHOBSTETRIC AND GYNECOLOGICAL CARE, EAR-NOSE-THROAT AND ORTHOPEDIC SPECIALTY CARE. AMERICAN RED CROSS BLOOD DRIVES. ADVOCATE SHERMAN HOSPITAL PARTNERS WITH THE AMERICAN RED CROSS TO PROVIDE MONTHLY BLOOD DRIVES HELD AT THE HOSPITAL TO ADDRESS THE ONGOING CRITICAL BLOOD SUPPLY SHORTAGE. IN 2023, A TOTAL OF 17 BLOOD DRIVES WERE COORDINATED. A TOTAL OF 774 DONORS (INCLUDING 84 FIRST TIME DONORS) COLLECTED 660 UNITS OF BLOOD WHICH EQUATES TO APPROXIMATELY 1,980 LIVES SAVED.
PART VI, LINE 6: I. 2022 COMMUNITY HEALTH NEEDS ASSESSMENT AND 2023-2025 COMMUNITY HEALTH IMPLEMENTATION STRATEGIESALL ADVOCATE HEALTH CARE HOSPITALS' 2022 COMMUNITY HEALTH NEEDS ASSESSMENTS (CHNAS) UTILIZED A MIXED METHODOLOGY APPROACH TO COMPLETING A COMPREHENSIVE STUDY OF THE DEMOGRAPHICS AND HEALTH NEEDS OF THE COMMUNITIES WE SERVE. DATA UTILIZED TO COMPLETE THE CHNA REPORTS INCLUDE PRIMARY AND SECONDARY DATA AND BOTH QUALITATIVE AND QUANTITATIVE DATA. AS ENCOURAGED BY THE IRS, EVERY HOSPITAL COLLABORATES WITH OTHER HEALTH CARE PROVIDERS, LOCAL HEALTH DEPARTMENTS AND OTHER COMMUNITY STAKEHOLDERS TO COMPLETE THE CHNA PROCESS EVERY THREE YEARS. FOLLOWING COMPLETION OF THE CHNA REPORT, EACH HOSPITAL DEVELOPS COMMUNITY HEALTH IMPLEMENTATION STRATEGIES (CHIS) TO DESCRIBE HOW THEY PLAN TO ADDRESS PRIORITIZED COMMUNITY HEALTH NEEDS.IN ILLINOIS, NINE HOSPITAL CHNA REPORTS WERE COMPLETED ON AN ALIGNED THREE-YEAR CHNA TIMELINE. ADDITIONALLY, TWO ADVOCATE CHILDREN'S HOSPITALS (OAK LAWN AND PARK RIDGE) ALSO COMPLETED A COMPREHENSIVE CHNA REPORT. ALL 11 CHNA REPORTS FOR ILLINOIS WERE REVIEWED AND APPROVED BY THE ADVOCATE HEALTH CARE NETWORK BOARD AND EACH HOSPITAL'S GOVERNING COUNCIL IN LATE DECEMBER 2022. BY THE FIRST WEEK OF MAY 2023, THE 2023-2025 COMMUNITY HEALTH IMPLEMENTATION STRATEGY PLAN FOR EACH HOSPITAL WAS MADE PUBLICLY AVAILABLE ON OUR WEBSITES. WHILE PROGRAMS CAN OFTEN SUPPORT MORE THAN ONE KEY FOCUS AREA, THE FOLLOWING EXAMPLES ARE ORGANIZED TO DEMONSTRATE FULFILLMENT OF ADVOCATE'S STRATEGY IN EACH FOCUS AREA AND PREFERABLY IN THE AREA IN WHICH PROGRAMS HAVE THE MOST IMPACT. ACCESS TO INNOVATIVE CARE AND SERVICE: ACCESS TO HEALTH CARE WAS RANKED AS A TOP ISSUE BY COMMUNITY MEMBERS AND KEY STAKEHOLDERS IN THE MOST RECENT COMMUNITY HEALTH NEEDS ASSESSMENTS FOR ALL ADVOCATE HEALTH HOSPITALS IN ILLINOIS AND WISCONSIN. ACCESS TO INNOVATIVE CARE AND SERVICES IS ABOUT CREATING CONDITIONS IN OUR HOSPITALS, ACROSS OUR HEALTH SYSTEM AND IN OUR COMMUNITIES TO PROVIDE ACCESSIBLE, AVAILABLE, AFFORDABLE, AND TARGETED HEALTH CARE AND HEALTH SERVICES TO SUPPORT EVERYONE'S ABILITY TO LIVE WELL. ACCESS TO CARE OFFERS AN OPPORTUNITY TO DETECT AND TREAT DISEASE AT AN EARLIER STAGE, IMPROVE OVERALL HEALTH, PREVENT DISEASE AND DISABILITY, AND REDUCE PREVENTABLE DEATHS. PATIENTS EARNING UP TO SIX TIMES THE FEDERAL POVERTY LEVEL (FPL) AND INSURED PATIENTS EARNING UP TO TWO AND HALF TIMES THE FPL, MAY QUALIFY FOR A FULL OR PARTIAL FINANCIAL ASSISTANCE DISCOUNT. ADDITIONALLY, A CATASTROPHIC ASSISTANCE DISCOUNT IS AVAILABLE FOR UNINSURED AND INSURED PATIENTS WHOSE INCOMES EXCEED THE TRADITIONAL FINANCIAL ASSISTANCE INCOME GUIDELINES AND HAVE OUTSTANDING PATIENT BALANCES OF $25,000 OR MORE FOR A SINGLE DATE OF SERVICE OR SUM OF SEVERAL DATES OF SERVICE. THESE PATIENTS MAY QUALIFY TO RECEIVE A FINANCIAL ASSISTANCE DISCOUNT THAT REDUCES THEIR OUTSTANDING BALANCE TO 25% OF THEIR NET INCOME. FOR UNINSURED PATIENTS, ADVOCATE WILL PRESUMPTIVELY PROVIDE FINANCIAL ASSISTANCE IF THE FINANCIAL STATUS HAS BEEN VERIFIED BY A THIRD PARTY. IN THESE CASES, THE PATIENT IS NOT REQUIRED TO SUBMIT A SEPARATE CHARITY APPLICATION. IF PRESUMPTIVE CRITERIA ARE NOT AVAILABLE FOR UNINSURED PATIENTS, FINANCIAL ASSISTANCE ELIGIBILITY IS AVAILABLE USING AN INCOME-BASED SCREENING. ADVOCATE EXTENDS ITS INCOME-BASED FINANCIAL ASSISTANCE POLICY TO ITS INSURED PATIENTS AS WELL. THE ADVOCATE THEN ASSISTS PEOPLE IN APPLYING FOR THE FINANCIAL ASSISTANCE PROGRAMS MOST APPROPRIATE FOR THEM. IN 2023, FINANCIAL ADVOCATES PROCESSED 14,565 ADVOCATE FINANCIAL ASSISTANCE APPLICATIONS, COMPLETED 2,817 MEDICAID APPLICATIONS, IDENTIFIED 788 PEOPLE FOR CO-PAY ASSISTANCE AND ASSISTED PEOPLE WITH COMPLETING 243 MARKETPLACE APPLICATIONS. FEDERALLY QUALIFIED HEALTH CENTERS (FQHCS). ALL ADVOCATE'S HOSPITALS CONTINUE TO HAVE RELATIONSHIPS WITH FQHC'S OR OTHER COMMUNITY CLINICS WITHIN THEIR SERVICE AREAS AND COLLABORATE WITH THOSE PARTNERS TO IMPROVE ACCESS TO CARE FOR MEDICAID AND UNINSURED PATIENTS. ADVOCATE SHERMAN CONTINUES TO WORK CLOSELY WITH GREATER FAMILY HEALTH (FQHC), VNA HEALTH CARE AND AUNT MARTHA'S (FQHC) TO COORDINATE CARE FOR LOW-INCOME PATIENTS IN THE ELGIN AREA. THE HOSPITAL PROVIDES COLONOSCOPIES AND MAMMOGRAMS TO GREATER FAMILY HEALTH PATIENTS, COORDINATED THROUGH GRANT-FUNDED PROGRAMS. ADVOCATE CONDELL WORKS COLLABORATIVELY WITH THE LAKE COUNTY HEALTH DEPARTMENT AND COMMUNITY HEALTH CENTER (FQHC) AND ERIE HEALTHREACH WAUKEGAN (FQHC), BY PROVIDING MAMMOGRAMS AND SOME SPECIALTY CARE TO UNINSURED AND LOW-INCOME PATIENTS AS THEY ARE REFERRED TO THE MEDICAL CENTER. ADVOCATE ILLINOIS MASONIC AND ADVOCATE LUTHERAN GENERAL ALSO PARTNER WITH HEARTLAND HEALTH CENTERS AND COMMUNITY HEALTH, ONE OF THE LARGEST FREE CLINICS IN THE NATION, TO PROVIDE SPECIALTY CARE TO UNINSURED PATIENTS AND REFERRALS TO FQHCS AND FREE CLINICS FOR PRIMARY CARE SERVICES. IN ADDITION, ADVOCATE ILLINOIS MASONIC PROVIDES OPERATIONS SPACE TO HEARTLAND HEALTH CENTERS ON ITS CAMPUS TO PROVIDE PRIMARY CARE FOR INDIVIDUALS AND FAMILIES THAT ARE UNINSURED. IN PARTNERSHIP WITH THE ACCESS TO CARE ORGANIZATION, ADVOCATE CHRIST CONTINUES TO PROVIDE FREE MAMMOGRAMS TO UNINSURED AND LOW-INCOME INDIVIDUALS THAT ARE REFERRED BY THEIR CLINIC TO THE HOSPITAL WHEN THIS SERVICE IS REQUIRED. ADVOCATE TRINITY WORKS WITH CHICAGO FAMILY HEALTH CENTER AND CHRISTIAN COMMUNITY HEALTH CENTER TO COORDINATE CARE FOR LOW-INCOME PATIENTS. ADVOCATE HOSPITALS IN COOK COUNTY WORK CLOSELY WITH COMMUNITY HEALTH TO CONNECT AND TREAT UNINSURED PATIENTS AND TO CONNECT INDIVIDUALS THAT NEED A PRIMARY CARE PROVIDER. TO MAINTAIN QUALITY CARE AND IMPROVE QUALITY OF LIFE FOR PEOPLE SEEKING CARE FROM ADVOCATE, WORKING TO FIND MEDICAL HOMES AND TO REDUCE EMERGENCY ROOM VISITS AND HOSPITAL ADMISSIONS IS ESSENTIAL. THESE ADVISORS WORKED CLOSELY WITH THE ORGANIZATION'S FINANCE, TAX, AND LEGAL ASSOCIATES AND OTHER MEMBERS OF THE ORGANIZATION'S TEAM ASSEMBLED TO PARTICIPATE IN THE PREPARATION OF THE FORM 990. THE FORM 990 IS REVIEWED BY FINANCE MANAGEMENT, THE TAX MANAGER, THE VP OF FINANCE/CORPORATE CONTROLLER, THE CHIEF FINANCIAL OFFICER, AND ADVOCATE'S OUTSIDE TAX CONSULTING FIRM AND TAX LEGAL COUNSEL. PRIOR TO PRESENTING THE FORM 990 TO THE BOARD OF DIRECTOR'S AUDIT COMMITTEE IN NOVEMBER, THE ORGANIZATION'S TEAM--INCLUDING ITS ADVISORS--MET FREQUENTLY TO DISCUSS AND REVIEW DRAFTS OF THE FORM 990. AT THE NOVEMBER AUDIT COMMITTEE MEETING, THE VP OF FINANCE/CORPORATE CONTROLLER AND CHIEF FINANCIAL OFFICER COORDINATED A REVIEW OF THE FORM 990 WITH COMMITTEE MEMBERS, AS THE AUDIT COMMITTEE IS THE COMMITTEE OF THE BOARD OF DIRECTORS CHARGED WITH OVERSIGHT OF AUDIT AND TAX MATTERS. FORM 990, PART VI, SECTION B, LINE 12C CONFLICT OF INTEREST POLICY THE ORGANIZATION'S CONFLICT OF INTEREST POLICY APPLIES TO VARIOUS PEOPLE, INCLUDING MEMBERS OF ADVOCATE'S BOARD OF DIRECTORS, GOVERNING COUNCILS, OFFICERS, ASSOCIATES, VOLUNTEERS, AND MEDICAL STAFF MEMBERS WITH ADMINISTRATIVE RESPONSIBILITIES. ANNUALLY, THE COMPLIANCE DEPARTMENT SENDS THIS POLICY AND THE ADVOCATE CODE OF BUSINESS CONDUCT TO A RANGE OF INDIVIDUALS WHO MAY BE IN A POSITION TO EXERCISE SUBSTANTIAL INTEREST OVER A PARTICULAR MATTER (DEFINED AS "INTERESTED PERSONS"). THEY ARE REQUIRED TO READ THE POLICIES AND PROVIDE THE COMPLIANCE DEPARTMENT WITH A DISCLOSURE STATEMENT, WHICH IDENTIFIES ACTIVITIES AND RELATIONSHIPS THAT COULD POTENTIALLY GIVE RISE TO A CONFLICT OF INTEREST. THE CHIEF COMPLIANCE OFFICER REVIEWS THE DISCLOSURES AND PROVIDES A REPORT TO THE SYSTEM BUSINESS CONDUCT (COMPLIANCE) COMMITTEE, EXECUTIVE MANAGEMENT TEAM AND THE AUDIT COMMITTEE OF THE BOARD FOR REVIEW. IN THIS STRATEGY, SPECIFIC MARKET COMPENSATIONS ARE IDENTIFIED AND THE DESIRED LEVEL OF COMPETITIVENESS IN THOSE MARKETS SPECIFIED. IN ADDITION, THE LINKAGE OF EXECUTIVE PAY TO PERFORMANCE IS ARTICULATED AND HOW THIS RELATIONSHIP IS TO BE MAINTAINED IS OUTLINED. TO SUPPORT AND IMPLEMENT THE COMPENSATION STRATEGY, FIVE BASIC ELEMENTS ARE UTILIZED.
