This statement is made under penalties of perjury, setting forth all the facts alleged as reasonable cause. An extension to the filing of the Form 990 Tax Year 2021 for our organization was not completed by the organization as it was not realized by the organization that our filing had not occurred until notification was received from the IRS in late October 2023. While it is difficult to communicate on this topic, on May 29, 2022 my friend, fellow teammate, and fellow active board member, Lisa Hiefner, passed away at my feet at the very start of our last game of adult coed soccer of the Spring 2022 season. While I signaled for help and for someone to call 911, I saw her children and ran to them and held them while other adult coed soccer players performed CPR as we awaited an ambulance to arrive. When paramedics arrived, I then contacted our Association President who came to the fields to assist. While we held out hope that Lisa would recover, we later learned that Lisa had suffered a brain aneurysm and she become an organ donor on her official passing on June 2nd. I did not realize that when I left the fields that evening and over the next several days at the hospital with her family, that I was in shock, and while I tried to continue in my duties as Association Treasurer to the best of my abilities, I look back now a year later and see how out-of-sync our Financial Statements were. It wasn't until late October 2022 that I began therapy and was diagnosed with PTSD. I could barely attend Association events in the Fall of 2022 as Lisa's name was everywhere: we were hosting a season-long fundraiser in her honor (she would pay out of her own pocket if a player's family couldn't afford the registration fee) and the Association chose to have an LH patch affixed upon each player's jersey that year. I would often have to leave our Association meetings or activities early because I couldn't face the difficulty of remembering Lisa passing away at my feet. Even now, I am struggling to fight back tears and those memories. I write all of this to demonstrate why our filing for 2021 didn't occur. We did attempt and were prevented from filing electronically at the time due to our FEIN somehow becoming associated with North Texas State Soccer, and so our attempts to file were rejected due to the name mismatch. While I don't recall much more of the time following that May 2022 game and the start of this year, I was unaware that we did not finalize and physically mail our 2021 tax year return. Had I been aware prior to receiving the IRS letter notifying me of such, I would have immediately submitted those. It is with every intent of my being to help this association maintain timely and proper filings, as I have spent many hours to restore the Association's 501c3 status after revocation occurred in 2019 due to the prior Treasurer filing our returns with the FEIN incorrectly typed. I spent many hours correcting our filings and business name with the State of Texas. Following reinstatement, I spent many hours working on getting our Association name corrected with the IRS as somehow our FEIN became associated with the North Texas State Soccer Association group filing (which we don't participate in) and then somehow our FEIN became associated with the Burleson Soccer Association. I have given my time, free of charge, to learn as much as I could about IRS regulations so that I could help to ensure that the Bedford-Euless Soccer Association is able to continue to operate properly as a 501c3 organization, dedicated to serving the youth of our area in the beautiful game of soccer. To ensure that such an incident does not occur again in the future, our organization has created a shared Google drive account in which we will share and document our filings, including a tracking document so that we can track and monitor timely submissions of our 990 forms. Additionally, I am working with a QuickBooks "live expert" to learn more about our ability to complete our 990 returns through our QuickBooks account. I apologize for sharing such a difficult topic within this request. I am truly appreciative of your consideration of this request.
