The original filing was filed in accordance with the extension which was granted however I underwent brain surgery last year and was not aware that the filing had to be done electronically as we had always filed direct with IRS in Ogden Utah. Since the IRS did receive the 990 EZ before the end of the Extension date we humbly request that no penalties be assessed.
