PENNSYLVANIA BUSINESSES AND INDIVIDUAL TAXPAYERS HAVE PARTICIPATED IN THE EITC, OSTC AND PRE-K TAX CREDIT PROGRAMS OFFERED BY THE STATE DEPARTMENT OF COMMUNITY AND ECONOMIC DEVELOPMENT (DCED). THE SIGNIFICANT CONTRIBUTIONS AND PARTICIPATION PROVIDED BY THESE BUSINESSES AND INDIVIDUALS ALLOWED BLOCS TO AWARD OVER 17,000 CATHOLIC AND PRIVATE SCHOOL SCHOLARSHIPS TO NUMEROUS COUNTIES IN PENNSYLVANIA FOR FAMILIES IN FINANCIAL NEED. FOR THIS YEAR, BLOCS HAS WORKED WITH CLOSE TO 200 BUSINESSES AND OVER 1,700 INDIVIDUAL TAXPAYERS TO TAKE ADVANTAGE OF OVER $110 MILLION IN TAX CREDITS. HOWEVER, THE NEED FOR ADDITIONAL FINANCIAL ASSISTANCE REMAINS SIGNIFICANT AS BLOCS RECEIVED OVER 30,000 APPLICATIONS AND OVER $100 MILLION DOLLARS IN QUALIFIED NEED. BLOCS THROUGH THE SCHOLARSHIP COMMITTEE HAS ESTABLISHED AN EMERGENCY SCHOLARSHIP PROGRAM WHICH REVIEWS EMERGENCY REQUESTS FOR FINANCIAL ASSISTANCE FOR FAMILIES BASED ON EXTENUATING CIRCUMSTANCES THAT GREATLY AFFECT A FAMILY'S INCOME AND ABILITY TO PAY FOR EDUCATION. FOR THE CURRENT YEAR, BLOCS PROVIDED SUPPORT OF APPROXIMATELY $1.2 MILLION FOR APPROXIMATELY 1,000 STUDENTS. BLOCS HAS ALSO DEVELOPED THE URBAN ENDOWMENT INITIATIVE TO SUSTAIN TARGETED SCHOOLS IN POOR AND LOW PERFORMING SCHOOL DISTRICT AREAS. UNDER THIS INITIATIVE, NINE SCHOOLS WORK WITH BLOCS TO ADOPT SUCCESSFUL PRACTICES IN MARKETING AND MANAGEMENT. THE SCHOOLS MAINTAIN AN OPEN ENROLLMENT POLICY AND BLOCS ASSISTS THE SCHOOLS TO HELP MEET OPERATIONAL NEEDS THROUGH ENDOWMENT DISTRIBUTIONS. FORM 990, PART VI, SECTION B, LINE 11B A COPY OF FORM 990 WILL BE PROVIDED TO THE AUDIT AND FINANCE COMMITTEE OF THE BOARD PRIOR TO FILING WITH THE INTERNAL REVENUE SERVICE. THE ENTIRE BOARD WILL RECEIVE A COPY OF THE FORM 990 AFTER IT HAS BEEN FILED WITH THE INTERNAL REVENUE SERVICE.
FORM 990, PART VI, SECTION B, LINE 12C BUSINESS LEADERSHIP ORGANIZED FOR CATHOLIC SCHOOL'S BOARD OF DIRECTORS ANNUALLY REVIEW THE CONFLICT OF INTEREST POLICY AS A GOVERNING BODY. THE MONITORING AND ENFORCEMENT OF THIS POLICY IS PERFORMED BY THE EXECUTIVE COMMITTEE AS PART OF THEIR RESPONSIBILITIES. BLOCS HAS THE BOARD ANNUALLY COMPLETE THE CONFLICT OF INTEREST DOCUMENTATION AND KEEP THEM WITH SIMILAR BOARD INFORMATION SUCH AS MINUTES, RESOLUTIONS AND THE LIKE.
FORM 990, PART VI, SECTION B, LINE 15 EXECUTIVE DIRECTOR COMPENSATION IS APPROVED ANNUALLY BY THE EXECUTIVE COMMITTEE OF THE BOARD, AND PERFORMANCE REVIEWS ARE DONE ANNUALLY BY THE BOARD CHAIRMAN. APPROVAL OF COMPENSATION IS DOCUMENTED IN THE BOARD MINUTES FOR THE EXECUTIVE DIRECTOR'S COMPENSATION. THE COMPENSATION COMMITTEE REVIEWED SALARIES OF SIMILAR POSITIONS AT SIMILAR SIZE ORGANIZATIONS TO DETERMINE A SALARY RANGE. FOR ALL OTHER EMPLOYEES, THE BOARD APPROVES COMPENSATION ANNUALLY AS PART OF THE BUDGET PROCESS PERFORMANCE AND COMPENSATION REVIEWS ARE PERFORMED BY THE EXECUTIVE DIRECTOR.
FORM 990, PART VI, SECTION C, LINE 19 THE ORGANIZATION MAKES ITS GOVERNING DOCUMENTS AND CONFLICT OF INTEREST POLICY AVAILABLE UPON REQUEST. THE ORGANIZATION ALSO MAKES ITS FINANCIAL STATEMENTS AVAILABLE ON ITS WEBSITE AS WELL AS UPON REQUEST FOR THE SAME PERIOD OF DISCLOSURE AS SET FORTH IN SECTION 6104(D).
