Explanation

Pt I Line 2

Childrens Hospital of Michigan Foundation (CHMF) is a well-respected and IRS recognized 501(c)(3) public charity (EIN 32-0087353). When we submit contributions/grants, the CHMF "General Grant Request Form" allows us to select the fund(s) we desire. We chose the pediatric epilepsy and pediatric heart funds. The grants are used by CHMF for research and treatment. We later receive letters of acknowledgement with thew statement "Any grant by the Fraternal Order of Eagles will be used for direct costs for (fund chosen) only and will not be used for administrative expenses."

Pt VI, Line 2	Two trustees married to each other
Pt VI, Line 6	Our organization has members. Currently we have 760 members. We do not have shareholders.
Pt VI, Line 7a	As a fraternal organization, our officers are elected by the membership. The Fraternal Order of Eagles has a structured procedure to follow in Nominations for Office and Election of Officers. The membership is notified by email, website, meeting announcements, and Social Room postings of the nominations and elections, and the procedures and requirements for both. In some cases of vacancies in office, the President can appoint, but even then may do so only with membership vote of approval to appoint.
Pt VI, Line 7b	Decisions are made by a vote of the membership. Motions are made, seconded, discussed, and then voted upon by the members present, in agreement with Robert's Rules of Order.
Pt VI, Line 11b	Members are advised via email, website, regular membership meeting announcements, and Social Room postings that Form 990 is available for member inspection prior to filing Form 990, and at any time thereafter. A copy of Form 990 is also sent to our headquarters, Grand Aerie, Fraternal Order of Eagles, Grove City, Ohio for their inspection, use, and verification of our operational compliance.
