Form 990, Part VI, Section A, Line 2	George Barlett, Director, is the father of Richard Barlett, President
Form 990, Part VI, Section A, Line 6	The Lodge is controlled and supervised by the members of the Lodge. All expenditures must be approved, by vote, before any expenditures are made. The Lodge members direct and control all matters and activities of the Lodge.
Form 990, Part VI, Section A, Line 7a	Members of the organization control and run the Lodge by electing the Board of Directors, who are responsible for the preservation of the Lodge properties, including real estate and investment funds.
Form 990, Part VI, Section A, Line 7b	The Lodge members run and control all matters of the Lodge. The Officers are mere agents of the Lodge and subject to the majority Lodge vote of its members.
Form 990, Part VI, Section B, Line 11b	Form 990 is provided to and reviewed by the Board of Directors before filing. The form 990 is submitted to the Lodge Secretary and becomes part of the permanent Lodge Minutes.
Form 990, Part VI, Section B, Line 12c	President and the Board of Directors monitor financial transactions and balances on a monthly basis. Any abnormalities are questioned and further investigated, if warranted.
Form 990, Part VI, Section B, Line 15	Compensation amounts for all Lodge Officers and other individuals, are presented to the organization's members.
