What is the organization's primary exempt purpose? Health Poverty Action USA acts in solidarity with health workers, activists and communities worldwide to improve health and challenge the causes of poverty. We do this in 3 distinct ways: We approach health as an issue of social justice. We recognize that the greatest causes of poor health worldwide are political, social and economic injustices. That is why we tackle the root causes of poor health as well as the symptoms. We prioritize those missed out by others. Development organizations tend to cluster together which leaves large populations with almost no support at all. We make these most neglected populations our highest priority. We address the full range of factors which impact on health. As well as strengthening health services we work on areas such as nutrition, water, sanitation, immunisation, and income generation. Tackling one cause of poor health in isolation can give the appearance of improving health, but in reality might do little more than change the cause of death.

28 Health Poverty Action USA is improving the health of mothers and young children in the western highlands of Guatemala. Below details the impact this project has had on the indigenous families living in some of the most remote and neglected areas of the country. The indigenous communities in the seven target districts, within Totonicapán and Quetzaltenango Departments, face numerous challenges accessing basic health services, including poor quality services, cultural and linguistic barriers and low levels of health awareness. Our intervention aims to improve the quality and utilization of maternal and child healthcare services serving women of childbearing age and children aged 0-<5 years, and to improve the nutritional status of pregnant women and mothers, in the seven districts (Momostenango, Santa Lucia la Reforma, Santa María Chiquimula, San Carlos Sija, Cabricán, San Vicente Buenabaj, Concepción Chiquirichapa). It has four key strands: improving the quality of services; improved coordination between Ministry of Health (MoH) and Traditional Birth Attendants (TBAs) to reduce maternal mortality and complications; and hospitals); increasing peoples health-seeking behaviour; and improving nutrition. This year, we have directly supported: - 914 pregnant women and 516 mothers of children under 5 with information on the importance of antenatal care, vaccination schemes, emergency planning, newborn care and nutrition. - 617 Traditional Birth Attendants (TBAs) through training sessions on emergency planning, danger signs during pregnancy and childbirth, care of the newborn, and the importance of antenatal care. They were also trained in misoprostol use, perinatal depression and nutrition. - 582 Ministry of Health (MoH) staff through training to improve the quality of antenatal care, training on culturally appropriate attention; and inclusive and participative community outreach methodology. We provided essential medical equipment to the targeted health facilities including to protect the vaccine cold chain, to enable monitoring of fetal heart rates and oxygen saturation of mothers and babies, amongst many items. We have also provided materials to improve water, sanitation and hygiene facilities.

29 Health Poverty Action USA works alongside local partners and many communities where people struggle to access sufficient essential health services, including displaced communities living in areas affected by conflict and hard to reach areas on the Myanmar side of China-Myanmar border. This project focuses on malaria and tuberculosis (TB), which is closely associated with poverty. China-Myanmar border countries are the most undeveloped in Myanmar. Marginalized ethnic minority groups living in the forest and forest fringes often carry the greatest burden of both poverty and disease. HPA USA aims to improve accessibility and quality of malaria and TB control services for migrants, as well as people living in endemic areas of the six Special Regions of Myanmar - including outreach, prevention, diagnosis, referrals and treatment - by training and supporting volunteer health and malaria workers and equipping health facilities. Project outcomes: - 649 integrated community malaria volunteers, 107 malaria stations, 43 private clinics, 6 mobile teams were supported for malaria case management in the past year. - 14,750 long-lasting mosquito nets were distributed to at-risk populations through mass and continuous campaigns with households and beneficiaries. - 25,133 confirmed malaria cases received first-line antimalarial treatment according to national policy at community and private sectors. - 1,315 presumptive TB cases were referred by community health workers/volunteers and health facilities to get examination in TB centers/hospitals. - 348 diagnosed TB cases received TB treatment. - 103 TB volunteers were trained and supported.

30 Health Poverty Action USA (HPA USA) is working with local partners to improve the health of pastoralist women, children and adolescents in Dolo Bay, Ethiopia. These funds and costs represent a small portion of the total funding/expenses for the project reported on here. In the Somali Regional State of Ethiopia within the south-east district of Dolo Bay, mobile pastoralist communities hold strong cultural values and live in very traditional settings. They migrate often when they need to search for fresh pasture for their herds. However, because they are always moving, this makes accessing quality health care extremely challenging, especially for pregnant women, babies and teenage girls. In fact, accessing maternal, child and sexual and reproductive health (SRH) services remains the biggest challenge in this region. This is because many people living in the pastoralist area of Dolo Bay believe and prefer to seek traditional healers over modern health care. In order facilitate access to health care, increased health seeking behavior in the target area as well as more training for health workers to develop the skills required in providing quality services is needed. Summary of outcomes: Improved Health Workers Capacity: The project has been able to train 117 health care workers in order to enable them to deliver quality healthcare services to mothers, women, children and girls. This includes the improvement of knowledge and skills of the health care workers, health extension workers, community health volunteers and some members of the community. Community Mobilization and Awareness Raising: Community mobilization and awareness creation has been the main tool utilised for improving the health seeking behavior of the community. To this end, community dialogues across the woreda were being performed on a monthly basis. These dialogues include discussion about gender-based violence, female genital mutilation, maternal and child health issues and several others were included by designing a comprehensive guidance about the community dialogues. A total of 11,270 people have been reached thanks to these activities. Improved Response to Domestic and Gender- based Violence (GBV): 6 community groups were established in the target woreda. 3 women groups and 3 girls clubs were formed in the community and school respectively. These groups are also being used as the primary owners of the GBV reporting mechanism created in the community. Key members of the community are also participating in the monthly discussions about critical issues of SRH, GBV, maternal and child health and others. These stakeholders include police, military representatives, religious leaders, women representatives and youth representatives. There have been 19 dialogues discussing domestic and GBV, 15 health workers made capable of managing SGBV prevention and reporting, 43 reported cases of SGBV in the schools and in the target community, and 18 SGBV victims supported through medical, psychosocial, legal services and transportation costs. In total, across outreach health work, mobile clinics, static health service and improved referral systems, we have provided quality health care to 18,289 women, adolescent girls, and children.
