Briefly describe the organization’s mission: The South Florida Dentists Self-Insurance Trust is a dental malpractice self-insurance trust providing primary malpractice coverage to dentists who are members of the trust. Membership is limited to dentists who are duly licensed by the State of Florida and who practice in the counties of Dade Broward Collier Monroe and Palm Beach Counties.

Part VI, Line 6	The South Florida Dentists Self-Insurance Trust is a Dental Malpractice Self-Insurance Trust providing primary malpractice coverage to Dentists who are members of the Trust. Membership is limited to Dentists who are duly licensed by the State of Florida and who practice in the counties of Dad Broward Collier Monroe and Palm Beach counties. All applications for membership are madden a written application form provided by the Board of Trustees. Approval for membership by the Board of Trustees requires a vote of the Board.
Part VI, Line 7a	The affairs of the South Florida Dentists Self-Insurance Trust are managed and its corporate powers exercised by a Board of Trustees composed of not less than 5 and no more than 7 members. Currently thee are 5 members. A nominating committee made up of one trustee and four general members who submit to the members a slate of trustees to fill the offices which are open each year. Individual members of the Trust not on the nominating committee may make nominations for trustees also.
Part VI, Line 11b	Once the Form 990 has been finalized by the accountant it is reviewed approved signed and filed by the South Florida Dentists Self-Insurance Trust.
Part VI, Line 12c	All of the organizations's financial activities are reviewed to determine if any transactions are not in compliance with the conflict of interest policy. There have been no transactions which are not in compliance with the conflict of interest policy.
Part VI, Line 19	No documents are made available to the general public as there is no requirement to do so. This entity is not a public charity but rather a small company insurance captive.
