Line 1	Form 990-PF, Part VIII-A	ACCELERATE THERAPIES The Three Lakes Consortium for Pulmonary Fibrosis (TLC4PF) was granted a no cost extension through the end of 2023. Final reports were received in February of 2024. The consortium included investigators from Yale School of Medicine, University of Pittsburg, McGill University, University of Southern California, and Baylor University. The consortium expanded the IPF Cell Atlas, improved the understanding of the precision-cut lung slice model, and developed a unique organoid model. The B4PF1.0 project was scheduled to finish in early 2024 but was extended throughout the year to allow for more complete analysis of the data. Preliminary data from the project showed that molecular changes are present prior to any evidence of interstitial lung disease on HRCT scans. The project is being continued with a large government grant. A grant was provided to Vanderbilt, Cornell and Fluidda to complete the B4PF2.0 project in 2024. In this project, HRCT scans from relatives of patients with IPF will be analyzed to develop an algorithm to identify people at risk of disease progression. Due to a challenge with de-identification of the scans, the project timeline has been extended. Ultimately, the results of the study should lead to a non-invasive test to identify risk for IPF and enable early preventative treatment. In 2024, the Three Lakes Foundation (TLF) continued to fund the Pulmonary Fibrosis Foundation biomarker consortium PROLIFIC which has validated assays to measure serum proteins linked to IPF prognosis. The funding in 2024 allowed partners access to the raw data and enabled more complete analysis of the data.
Line 2	Form 990-PF, Part VIII-A	IMPROVE TIME TO DIAGNOSIS The TLF grant period for The American College of Chest Physicians (CHEST) to promote The Bridging Specialties tool kit concluded in 2024. This grant period included the launch and implementation of a direct to physician awareness and outreach campaign with strategic distribution partners including Healio, FreeCME, myCME, and the AMA's Ed Hub. The toolkit CME activities went live on these platforms throughout 2024. As part of the CHEST grant, two $150,000 grants were awarded to the University of West Virginia and the University of Chicago to implement the Bridging Specialties- Timely Diagnosis for ILD toolkit and to assess the impact of the toolkit on improving time to diagnosis and the accuracy of diagnosis for patients with suspected ILD. The project to support a nurse navigator to refer people with ILA or ILD in lung CT scans to a Lung Health Clinic for follow-up at Lahey Clinic ended in April 2024. The program demonstrated that use of a Natural Language Processing (NLP) screen decreased time to diagnosis. The Mayo Clinic grant to screen electronic health records to understand the IPF patient journey and effects of socioeconomic factors, environmental factors, comorbidities and treatments on outcomes was completed in March 2024.
