OWENSBORO HEALTH TWIN LAKES MEDICAL CENTER ("OHTLMC") IS A 75 BED HOSPITAL BASED IN LEITCHFIELD, KY, ABOUT 65 MILES SOUTHEAST OF OWENSBORO. THE SERVICE AREA COMPRISES 115,000 PEOPLE FROM SIX COUNTIES: GRAYSON (PRIMARY), BRECKINRIDGE, OHIO, BUTLER, EDMONSON, HART, AND HARDIN. OWENSBORO HEALTH, INC. ("OH") WHOLLY OWNS AND CONTROLS OHTLMC AND IS NOT A MEMBER OF THE OBLIGATED GROUP. OHTLMC'S PRIMARY ACTIVITY CONSISTS OF PROVIDING MEDICAL AND PATIENT CARE SERVICES IN ITS MAIN HOSPITAL, SERVING SIX COUNTIES, IN SUPPORT OF OH. THE SOLE MEMBER OF OHTLMC IS OH. OUR MEDICAL SERVICES ARE DELIVERED BY HIGHLY SKILLED PHYSICIANS ALONG WITH A CARING AND COMPASSIONATE NURSING STAFF. IN MAY 2023, OHTLMC OPENED ITS NEW INPATIENT 12-BED GERIATRIC BEHAVIORAL HEALTH UNIT. THE UNIT IS SPECIFICALLY DESIGNED TO PROVIDE MENTAL HEALTH SERVICES FOR THE AGING POPULATION. THE GOAL IS TO HELP OLDER ADULTS EXPERIENCE LESS DISRUPTIVE BEHAVIORS AND SLOW THEIR RATE OF COGNITIVE DECLINE. FOR THE FISCAL YEAR 2025, OHTLMC HAD TOTAL ADMISSIONS OF 1,601, PATIENT DAYS OF 6,588 AND TOTAL ER VISITS OF 19,954. COST OF PARTICIPATING IN GOVERNMENT PROGRAMS: OHTLMC IS COMMITTED TO SERVING ALL PERSONS IN NEED, REGARDLESS OF RACE, CREED, SEX, NATIONALITY, RELIGION, DISABILITY, AGE OR ABILITY TO PAY. TO PROMOTE ACCESS TO CARE, OHTLMC PARTICIPATES IN THE FOLLOWING PUBLIC HEALTH PROGRAMS: MEDICAID, MEDICARE, TRICARE AND LOCAL HEALTH DEPARTMENTS. IN GENERAL, PAYMENTS FROM THESE PROGRAMS FREQUENTLY DO NOT COVER THE COSTS OHTLMC INCURS TO SERVE PROGRAM BENEFICIARIES. UNCOMPENSATED CARE AND FINANCIAL ASSISTANCE: OHTLMC PROVIDES MEDICAL CARE WITHOUT CHARGE OR AT REDUCED COST TO RESIDENTS OF THE COMMUNITIES THAT IT SERVES. In its most recent Community Health Needs Assessment (CHNA), Owensboro Health Twin Lakes Medical Center (OHTLMC) and its Community Health Committee with Blueprint Kentucky formerly, Community and Economic Initiative of Kentucky (CEDIK) developed a survey to gather health information and perceptions of Grayson County residents 18 years of age and older. The survey was conducted from August-November 2024 using an online tool and distributing printed copies in various locations throughout the community. The online survey was promoted through local media, emails, and its Facebook page. Emails with a link to the survey were sent to Grayson County Chamber of Commerce members, local industries, and employees of the Grayson County school system and the hospital. Printed copies were delivered to an assisted living facility, the Grayson County Alliance food bank, the Grayson County Health Department, Big Clifty Medical Complex, and the Caneyville Family Practice. 631 Grayson County residents participated in the survey. The survey questions were in three primary categories: demographics, personal health information, and perceived health needs of the community. Blueprint Kentucky tabulated the results and supplemented them with health statistics and demographic information. OHTLMC then invited representatives from various backgrounds to participate in focus groups. These focus groups targeted: the Community Health Committee, youth, social determinants of health, economics, seniors, and individuals with special needs represented included the county extension office; Grayson County Schools; the Grayson County Health Department; OHTLMC employees; local industry/manufacturing; Grayson County Healthcare Foundation; Catholic Outreach; Tri-County CASA; banks and businesses; Owensboro Health Regional Hospital; the Grayson County Alliance, special need providers, and senior citizens, and community members. Lastly, four key informant interviews were done with involved community members. After carefully considering the results of the Community Health Survey, the most recent health and demographic information available input from the Community Health Committee, the focus groups and the key informant interviews, the following needs were identified by the OHTLMC Community Health Committee and approved by the hospital leadership and the Board of Directors. The priority health areas for OHTLMC in tax years 2024 - 2027 are: Mental Health, Addiction (Legal/Illegal Substance/Behavior), Nutrition, and Physical Inactivity.

As previously noted, the Owensboro Health Twin Lakes Medical Center (OHTLMC) CHNA was conducted with Blueprint Kentucky. Blueprint Kentucky engaged multiple community partners, individual citizens, elected officials, coalitions and subject matter experts in conducting the CHNA.
Schedule H, Part V, Section B, Line 11 Facility , 1 Facility , 1 - OWENSBORO HEALTH TWIN LAKES MEDICAL CENTER. The OHTLMC Implementation Strategy Team, comprised of internal hospital team members, accessed supplemental data, expertise from service lines, departments, and resource information to better understand the priority health issues and possible strategies. This internal steering committee meets quarterly to update progress made on strategies and every three years to develop a new plan based on the findings of the most current CHNA. OHTLMC began screening all patients admitted as inpatient for social determinants of health on January 23, 2023. This is being utilized and allows OHTLMC case management/quality/community engagement and population health team members to identify better opportunities to link patients with resources related to their identified needs to provide a better quality of life. As identified priority health issues are correlated to social determinants of health, namely poverty, transportation, education, and nutrition, OHTLMC will be looking to expand upon internal and external strategies now used to address community health. In addition to the OHTLMC Implementation Strategy Team, Owensboro Health Twin Lakes Medical Center also created a Community Health Committee. The group is guided by the identified priority health needs named in the hospital's most recent CHNA identifying areas of focus to improve the health of the community and support the hospital's implementation strategy. The Community Health Committee is comprised of both internal team members and external community members. The Community Health Committee meets monthly to discuss work being conducted throughout the community to impact the identified priority needs and social determinants of health. Also, as a strategy, OHTLMC uses the CHNA report as a foundation to build lasting relationships between OHTLMC, community partners, and residents of Grayson County. OHTLMC has engaged vital internal and external community partners to address the prioritized needs. The strategies used to impact the priority health areas include, but are not limited to, modifying policies, public and private; providing support, information, and incentives; improving access; enhancing skills, and changing consequences. OHTLMC is working collaboratively with community partners to address the identified priority areas. Identified Priority Needs: Mental Health, Addiction (Legal/Illegal Substance/Behavior), Nutrition, Physical Inactivity OHTLMC will address all needs identified in its most recent CHNA. Mental Health: In addition to partnering with the local community mental health center serving Grayson County and other providers of mental health services, OHTLMC continues to operate its new inpatient 12-bed Geriatric Behavioral Health Unit, averaging a daily census of 6 patients. This additional service increased access to mental health services for the aging population in and around Grayson County. In addition to the inpatient services, OHTLMC offers outpatient behavioral health services to the community at large utilizing the Owensboro Health Regional Hospital inpatient behavioral health services, telehealth behavioral health interventions, and Intensive Outpatient Programs. OHTLMC has a behavioral health physician to provide outpatient services to the community. In calendar year 2024, 1503 visits were conducted, an increase from the calendar year 2023 which included 1231 visits. To ensure OHTLMC team members are equipped with the tools needed to provide a high standard of care for patients seeking mental health services, Mental Health First Aid is being offered to OHTLMC team members. As of May 2024, OHTLMC has 9 team members certified in mental health first aid; additional classes are expected to be offered in the following year. Mental health issues among youth and adults continue to be on the rise. OHTLMC works to explore strategies to discover innovative solutions for educational outreach. Since June 2023 OHTLMC continues to promote, through traditional/social media channels and local community organizations the availability of a population level mental health intervention, a vetted virtual resource called CredibleMind. In addition, the hospital hosted its fourth annual Mental Health and Addition Awareness Fair open to the community. 90 community members attended this community service with 50 mental health vendors providing mental health resources. OHTLMC participated in the second annual Grayson County Poverty Coalition Summer Resource Fair in June 2024. The fair consisted of 65 vendors, nine of which were Owensboro Health services, to provide community and basic need resources to 380 participants to link the community to available resources they may otherwise been unaware of. This is an annual event that OHTLMC continues to work towards having a growing presence. Future plans identified in the 2024 implementation strategy OHTLMC intends to strategically address mental health are: continue distributing education and increasing knowledge of a population level mental health intervention, a vetted virtual resource, CredibleMind by working with the Grayson County School System and other community partners and also including information about this resource on all after visit summaries given to patients at OHTLMC; implement a social media campaign highlighting the importance of healthy sleep habits; develop and implement a community walking initiative focusing on relationship building and socialization. Substance Use: Since April 2019, OHTLMC has continued to be a member of the Kentucky Opioid Response Effort's Kentucky Statewide Opioid Stewardship Program (KY-SOS) to address inappropriate or excess opioid prescribing. The Opioid Stewardship Team at OHTLMC meets regularly and participates in data collection and reporting as required by KY-SOS. In addition, OHTLMC participates in the established Opioid Stewardship Committee within the Owensboro Health system, whose charge is to ensure safe opioid prescribing and decrease opioid abuse and misuse by patients in the hospital's care. Treatment: OHTLMC offers medical detox from alcohol, opioids, and other addictive substances. In 2024 68 consultations were conducted, an increase from 39 in 2023. OHTLMC Community Engagement Coordinator serves as a community conduit to provide education and information regarding the prevalence of substance use disorder and education, when possible, to the community regarding substance use prevention. This person serves on the Grayson County Substance Abuse Focused Education (SAFE) Coalition, the Grayson Hardin Meade Agency for Substance Abuse Policy (GHM ASAP) Coalition, hosts a monthly substance abuse providers' meeting, "Taking Back Kentucky, hosts an annual Mental Health and Addiction Resource Awareness Fair which was attended by 90 community members in 2024 and included 50 substance use /mental health vendors distributing resources. (Continued) In March of 2023 OHTLMC began distributing self-help recovery workbooks to patients admitted for any of the hospital's addiction services program. These books will be utilized by patients over the 3-5-day period while in the program to begin to work through addiction related trauma, relapse triggers, and many other key factors important for their recovery. The Community Engagement Coordinator at OHTLMC also assisted in the development of an OH system- wide substance abuse focus team that met for the first time in April 2023. The vision for this team is that Owensboro Health develops and supports a comprehensive plan to change the culture of care surrounding substance abuse disorder through our commitment across the system providing education, prevention and treatment to the communities we serve. This team has had multiple successful initiatives since its inception: increased awareness among providers across the system to be aware of addiction services provided at OHTLMC for referral opportunities; started the work to create a resource webpage for patients, visitors and the community at large to utilize when seeking substance use resources; worked with community partners to identify a need for Narcan dispensing within the communities and currently aided in placing six Narcan dispensing boxes across the community totaling 638 boxes of Narcan being distributed in the community. OHTLMC has provided staff to assist the community in efforts to achieve a Kentucky Recovery Ready Community Certification. OHTLMC worked with the Grayson County Schools to explore opportunities to bring evidence-based substance use education into the school system. Plans are in place to launch an evidence-based curriculum, "Too Good for Drugs" in 2025 and OHTLMC will provide staff to be a part of a team to educate all students in grades 6-8. OHTLMC was able to certify one team member as a tobacco treatment specialist (TTS) in August of 2023. This team member is now doing consultations on inpatient admissions, connecting them with nicotine replacement therapy and referring them for follow up care. OHTLMC Trauma Services, in conjunction with Community Engagement and community partners, plan to develop and host an annual ghost-out event during prom season to educate students on the dangers of impaired driving and promote safe and responsible decision making. Future plans identified in the 2024 implementation strategy OHTLMC intends to strategically address substance use are: to seek and receive grant funding to expand the number of Narcan dispensing stations located in Grayson County; ensure all patients who are prescribed a controlled substance receive information on safe and appropriate medication disposal, including designated disposal sites available within the community; surpass the state benchmark or improve from baseline by 10% or more in opioid prescribing for co-prescribing and non-complicated vaginal deliveries for calendar year 2025 and all vaginal deliveries in calendar year 2026; develop a commitment pledge to refrain from distracted driving for OH team members and community at large and educate as needed; develop commitment pledge to establish appropriate limits of social media usage and education as needed; allocate staff resources to support the establishment of an Overdose Fatality Review Committee in Grayson County and provide personnel to actively participate in the committee meetings as needed; seek to make medications more affordable in partnership with Kentucky Prescription Assistance Program (KPAP). Physical Inactivity: OHTLMC has developed a partnership with Leitchfield Parks and Recreations and placed health fact signage at a local park. Phase 2 of this project will include directional maps to display all walking trails and distances. OHTLMC plans to expand this project to include other community parks over the coming year. From January to May of 2023, OHTLMC began the foundational work to develop a community walking initiative to raise awareness of Grayson County residents' personal fitness and activity levels. The planning work outlined goals to educate and engage all ages and demographics of Grayson County residents by providing and promoting local resources to improve health and fitness for individuals, promoting and educating the benefits of movement with health facts and statistics, and developing partnerships within the community that will provide increased opportunities for physical activity. For this project, OHTLMC secured partnerships with the City of Leitchfield, Leitchfield Parks and Recreation, The City of Clarkson, Grayson County Public Trails Association, and Grayson County High School. A Grayson County High School senior created the program's brand image between March and May 2023. From June 2023- May 2024, OHTLMC launched their fitness initiative with great success. At the end of tax year 2024 this initiative, the "Fitt Fiddle Program" included: attending 15 events, tracking 3711 community members impacted and recording 10,361 miles walked. OHTLMC will continue to expand this program to include programming in local elementary and middle schools, attending high traffic community events, developing a fan club for program participants, and developing and hosting an annual 5K event. Future plans identified in the 2024 implementation strategy OHTLMC intends to strategically address physical inactivity are: enhance employee engagement in wellness activities at OHTLMC by increasing participation in biometric screenings, the employee wellness program, and gym memberships discounts offered to employees; develop and implement a plan to increase awareness of the age friendly 4Ms initiative, with a focused emphasis on promoting mobility; leverage social media platforms to deliver consistent education on accessible at-home exercises. Nutrition: OHTLMC worked to create a series of pop-up farmers market events on site in partnership with the local farmers' market. This project aimed to provide easy access to healthy, locally sourced foods for team members and patients. OHTLMC plans to make this an annual event. OHTLMC is working with the Grayson County Alliance to develop a process to ensure patients identified as food insecure during a SDOH screening have access to non-perishable food items. OHTLMC currently hosts two annual holiday food drives for a local food pantry. Plans are being made to modify these events to focus on healthy food options. The hospital has signage at access points regarding financial assistance offerings. The hospital has financial aid applications available at registration areas, via the internet at the hospital website, via phone, and sent routinely via mail to patients of the hospital. The hospital provides financial counselors and contracts with an outside firm to ensure patients are interviewed and evaluated for eligibility in the financial assistance programs available. All self-pay and balance after insurance accounts are reviewed and worked by hospital representatives to ensure that the patient is given every opportunity to apply for financial assistance. Additionally information about applying financial assistance is included on the patient statements, bills, and letters and the patient guide they may receive from the hospital.

OHTLMC proves to be a driving force in change by educating the community on health & wellness issues, recognizing community needs, and working to fulfill those needs.
