Explanation:

Due to COVID in the tax years 2022 and 2023, our organization completed the 990-N Postcard internally. Prior to that, we had enlisted the services of the same CPA since 2012 to file the 990. In February of this year, we contacted this same CPA to complete the 990 since our income was over $50,000 for 2024. He agreed to complete the form for us but indicated that he could not work on it until after 4/15. We confirmed with him that he would file the extension due to this. We contacted the CPA after receiving a notice from the IRS on 9/29/2025 that we had not filed the 990 and he confirmed that he did not file the extension in error. We engaged the services of another individual to get the 990 filed as quickly as possible and accurately. We ask that no penalty be assessed to the organization for this cause and our past history of filing our 990 on time.
