INDICATES HOW AND WHEN ANY GRANT REPORTS REGARDING THE USE OF THE GRANT FUNDS SHOULD BE PROVIDED; AND IV.) ACKNOWLEDGES THE AUTHORITY OF MUTUAL OF OMAHA FOUNDATION TO WITHHOLD AND/OR RECOVER GRANT FUNDS IN CASE SUCH FUNDS ARE, OR APPEAR TO BE, MISUSED.
