SUPPLEMENTAL INFORMATION Schedule O Since its inception in 2000, the Highmark Foundation has made a lasting impact in the communities Highmark served by taking the steps necessary to improve the overall health and well-being of its residents. Throughout 2024, the Foundation continued to stand behind its mission of improving the health, well-being, and quality of life for individuals and communities by awarding a series of grants, sponsorships, and educational support, totaling $18.9 million. Since 2000, the Foundation has awarded over $175 million to charitable organizations, safety net providers, schools and hospitals across Pennsylvania and West Virginia. The Highmark Foundation has three key funding categories in place for its grant making efforts: * Chronic Disease * Family Health * Service Delivery Systems For over 20 years the Foundation has served as a catalyst and thought leader in addressing community health for the underserved, uninsured, Medicaid and Medicare populations throughout the Foundation's footprint of Pennsylvania and West Virginia. The Foundation awards grants in two ways: Invited Proposals and Request for Proposals. Over the past year, the Foundation has provided funding for: Cancer screening and community awareness campaign; free dental services; mental health; youth and vaping in schools; post-incarceration health; case management and care coordination, along with other important grants. In addition, the Highmark Foundation approaches grant making proactively, engaging the health care community, and utilizing research to identify areas of need in which Foundation funding will make a profound difference. The Foundation has also created urgency around many public health issues facing the region. It is achieving its mission by funding strategic and evidence-based health care initiatives that educate, protect, and ultimately improve the overall well-being of the large population it serves and has a three-pronged approach to its work: * Public Health * Systems Change * Thought Leadership This approach compliments the Foundation funding categories (mentioned above) as it seeks to support programs that reach vulnerable or at-risk individuals such as those who are uninsured, underserved, Medicaid and Medicare populations. These programs include pilot projects, model programs, illustrations of the effectiveness of early intervention and preventive health programs, incorporate sound programmatic methods and evaluations as building blocks for long-term program success and stimulate the efficient use of scarce health resources through cooperative planning and program initiatives which encourage cross-sector collaborations.
