Schedule D, Part V, Line 4 Intended uses of endowment funds The earnings are to be used for education, charity care, and other hospital programs. Swedish Covenant Health is the related beneficiary of the endowment funds held by Swedish Covenant Hospital Foundation.
Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnote The affiliates and subsidiaries of the System are primarily tax-exempt organizations under Internal Revenue Code Section 501(c)(3), and their related income is exempt from federal income tax. Accordingly, there is no material provision for income tax for these entities. Some of the income received by exempt entities is subject to taxation as unrelated business income. The System files federal income tax returns and returns for various states in the U.S. ASC 740-10, Income Taxes, requires that realization of an uncertain income tax position is more likely than not (i.e., greater than 50% likelihood of receiving a benefit) before it can be recognized in the consolidated financial statements. Furthermore, this interpretation prescribes the benefit to be recorded in the consolidated financial statements as the amount most likely to be realized assuming a review by tax authorities having all relevant information and applying current conventions. This interpretation also clarifies the financial statement classification of tax-related penalties and interest and sets forth new disclosures regarding unrecognized tax benefits. No amount was recorded for the year ended December 31, 2024. At year-end December 31, 2024, the System has a net operating loss carryforward of $416,494, which generated deferred tax assets of $121,678. These assets are offset by a valuation allowance of $112,623.

Community Health Survey Swedish Hospital developed a comprehensive online community health survey to accept input from people who represent the broad interests of the community served by the hospital. The survey, available in English and Spanish, asked participants to share their beliefs and perceptions about access to care, behavioral health, chronic disease/health issues, modifiable risk factors/behaviors and other concerns. It also asked the participants to rank their top health concern for the community. Community members who live in the hospital's service area were invited to participate in the online survey through social media, a system leader newsletter, and various emails to Swedish's donors, volunteers, employees and patients. The survey link, located on Endeavor Health's website (endeavorhealth.org/survey), was open March 25 - April 8, 2024 and 881 people in Swedish's service area responded. Key Informant Survey An additional online survey was implemented as part of the CHNA process specifically to solicit input from key informants. Key informants are defined as individuals who are considered experts in public health. Potential participants were chosen because of their ability to identify primary concerns of the community, including the medically underserved, low-income and minority populations served by the hospital. On March 4, 2024, the hospital President sent an email to the key informants to explain the purpose of the survey and invite them to participate. A direct link to the survey, which remained open until March 18, 2024, was also included. A reminder email was sent on March 14, 2024 to encourage participation. The key informants were asked the same questions as the community, focused on access to care, behavioral health, chronic disease/health issues, modifiable risk factors/behaviors and Social Determinants of Health and other concerns. It also asked them to rank their "top health concern" for the community. A total of 26 surveys were completed at Swedish's invitation. The Key Informant Survey was modeled off of the Community Survey, with minimal variations. Through this process, input was gathered from individuals whose organizations work with low-income populations, minority populations or other medically underserved populations (including the disabled, the elderly, the homeless, Medicaid/Medicare beneficiaries, the mentally ill, pregnant teens, substance abusers, undocumented individuals, veterans and uninsured/underinsured residents). These focus groups were held at The Friendship Center and Centro Romero in Chicago, located within the hospital's primary service area. Potential participants were given a short screening questionnaire to verify they lived in the hospital's service area and to ensure that participants selected would vary in age, gender, insurance status, income and educational levels, to the extent possible. The focus groups followed a guideline which mirrored questions included in the community and key informant surveys so that the information gathered could be used to compare with the survey results. The moderator summarized the results of the focus groups which were used as one of the tools in identifying the most predominant community needs

In consideration of the top health priorities identified through the CHNA process - and taking into account hospital resources and overall alignment with the hospital's mission, goals and strategic priorities - it was determined that Swedish Hospital would focus the majority of its efforts on developing and/or supporting strategies and initiatives that address the following priority areas (in no particular order), with health equity and SDOH woven throughout: 1. Chronic Health Conditions and Wellness In acknowledging the wide range of priority health issues that emerged from the CHNA process, Swedish determined that it could only effectively focus on those which it deemed most pressing, most under-addressed and most within its ability to influence. Swedish worked with key stakeholders to develop strategies, tactics and metrics for the majority of the top 50% prioritized needs identified in the CHNA. The remaining needs in the top 50% are addressed as noted below. - Homelessness and Housing (adequate, affordable, safe) - This need is addressed and integrated into the SDOH screening assessment. Swedish social workers have direct linkages with organizations addressing affordable housing. Swedish collaborates with numerous local not-for-profit organizations that support homelessness including Lutheran Social Services of Illinois. The Swedish Hospital Foundation also engages with Emergency Department staff to support housing needs. The hospital is committed to working with other organizations where possible to support these efforts and elevate awareness of these issues. - Older Adults Aging In Place - This need is addressed and integrated into the SDOH screening assessment. Swedish social workers have direct linkages and work to navigate older adults to necessary resources based on identified needs. Additionally, Swedish is launching a Sapphire Circle senior affinity program to provide ongoing education and support for local senior community members. - Domestic Violence and Community Violence - The Pathways Program at Swedish Hospital provides crucial support for patients experiencing domestic violence, human trafficking, and sexual assault, leveraging healthcare providers' roles to identify abuse and offer immediate support with safety planning, crisis intervention and advocacy at bedside. The program also links survivors to free trauma counseling and case management services on campus. Swedish partners and provides financial support to a number of not-for-profit organizations that address domestic and community violence including: Apna Ghar, Between Friends, The Network, Salvation Army's STOP-IT Program and others. In addition, Endeavor Health is a member of the Northwell Collaborative Gun Violence Prevention Learning Collaborative for Health Systems and Hospitals. - Adequate Employment Opportunities - Swedish Hospital is committed to raising awareness about employment opportunities at all levels within the health system, as well as creating opportunities for youth to learn about careers within health care. Due to resource constraints, Swedish is focused on other initiatives which we are able to most effectively impact. However, the hospital is committed to working with other organizations where possible to support these efforts, as well as supporting local community organizations via our Community Investment Fund. - Access to Oral Health Care - Swedish Hospital's Dental Clinic helps community members, including uninsured and low-income patients to access dental services. During 2024, the hospital took the following actions to address the key priority areas identified in the 2022 Community Health Needs Assessment: Access to Health and Social Services - SDOH Screening and Referrals - Expanded efforts to identify and respond to social drivers of health (SDOH) by implementing a screening referral tool in some inpatient and outpatient settings. In 2024, 12,641 screenings were completed, with 23% screening positive for one or more SDOH needs. A total of 3,057 internal and external referrals to resources or services were initiated. - Benefits Enrollment Support - Enhanced efforts to support benefits enrollment for under-resourced community members, including initiating a referral process to enroll patients who screen positive for food insecurity into programs like LINK, SNAP and WIC. In 2024, 304 individuals were connected to benefits enrollment resources through the Food Connections program. - Specialty Care and Diabetes Education - Increased access to specialty care and diabetes education via the Healthcare Transformation Program, partnering with local FQHCs to provide specialty medical services and access to individual diabetes education. In 2024, 1,509 new office visits and 3,141 follow-up visits were provided, including 878 uninsured new patients. There were a total of 307 nutrition and diabetes encounters. - Support for Survivors of Violence - Strengthened the capacity to respond to survivors of sexual and intimate partner violence via Pathways Program. In 2024, 926 team members were trained on topics such as the neurobiology of trauma, domestic violence, human trafficking and sexual assault. Additionally, 125 team members were trained in de-escalation techniques. The program responded to 266 crisis intervention requests and provided counseling and case management to 473 people. - Addressing Health Gaps - Developed community health strategies to address health disparities through partnerships and program development, playing a Regional Lead role for the North/Central Zone of Healthy Chicago Zones (HCZ) initiative. In 2024, HCZ provided 15 outreach events, with 1,055 total participants at the outreach events. Additionally, the team participated in 23 engagement events with 1,365 total participants at engagement events. - Community Wellness Center Programs - Offered educational programming, support groups and wellness events via the Community Wellness Center at Galter Life Center. In 2024, 79 programs were hosted, with a total of 148 sessions. Mental and Behavioral Health - Partnerships with Community Organizations - Strengthened partnerships with organizations addressing mental health, including continued collaboration with Lutheran Social Services of Illinois (LSSI) through inpatient and outpatient services on campus (Project IMPACT within the ED, The Welcoming Center and LSSI Mobile Crisis Team). This includes treatment for patients needing medical stabilization for substance or alcohol abuse. In 2024, 675 patients were served by Project Impact. - Integrated Behavioral Health Services - Continued and enhanced behavioral, mental health and substance abuse services, including a team-based approach that co-locates a clinical psychologist among primary care providers to better facilitate warm handoffs and streamlined connection to behavioral health services. - Bilingual Community Health Social Worker - A bilingual (English/Spanish) LCSW provides psychoeducation in the community and individual services. - Opioid Addiction Clinic - A suboxone opioid addiction clinic operates Wednesday evenings within the Emergency Department, with more than 100 visits to the suboxone clinic in 2024. - Family Connects Program - Supported Chicago families who have newborns through the Family Connects program, completing 528 post-natal home visits in 2024. - Support Groups for New Moms - Continued free weekly support groups for new moms, open to all community members. - Counseling Services - Provided more than $32,000 worth of free counseling services to uninsured or underinsured women via Swedish Hospital Foundation's Women's Care Fund. Chronic Health Conditions and Wellness - Preventative Screenings and Interventions - Addressed high blood pressure, diabetes and preventative cancer screenings through targeted interventions and outreach, including navigating patients with no record of mammogram history to Breast Health Center and providing funding for free screening as needed. In 2024, more than 1,400 no-charge cancer detection services were provided. Additionally, 84% of female patients from Swedish/NorthShore medical group aged 42-75 received a screening mammogram within the prior 24 months. - Blood Pressure and Diabetes Control - Controlled blood pressure in 75% of patients with hypertension within the Swedish/NorthShore medical group. 73% of patients with diabetes had A1c controlled. - Chronic Health Management - Enhanced partnerships with local organizations to address chronic health conditions, including deeper collaboration with FQHCs via the Transformation Program. In 2024, 693 appointments were completed for patients managing A1c and 618 appointments were completed for patients managing hypertension. - Community Education and Outreach - Expanded education and outreach, promoting nutrition, healthy lifestyles and wellbeing. Schedule H, Part II Community Building Activities Community Support-The hospital's nursing director from the Emergency Department represents the organization through active participation in the Chicago Healthcare System Coalition which covers various emergency preparedness topics and other emergency expertise. The nursing director serves on the coalition's executive committee and participates as an active coalition member. Coalition Building-The hospital convenes stakeholders semi-annually through a Community Leader Meeting. Attendees represent social service organizations, healthcare, education, faith communities, other nonprofits and elected officials. The meetings serve as space to learn about community health needs and opportunities for intervention and collaboration. It is also a space for the attendees to connect and share updates with one another. Advocacy for Community Health Improvement/Safety-Members of Swedish Hospital staff work with other organizations and participate in many activities to advocate for health improvement in our community, including access to care and safety net importance. These include: involvement with the association for community safety net hospitals and the IHA advocacy outreach team, community meetings, chambers of commerce, education, and seeking of resources to enhance health care services in the community. During the fiscal year, Swedish Hospital continued its involvement in the Alliance for Health Equity (AHE), a collaborative which includes partnership among the Illinois Public Health Institute, 30+ hospitals, 7 health departments, and community organizations across Chicago and Cook County. AHE promotes a collective impact on health outcomes in the city of Chicago through an environment that fosters learning, sharing of resources, data and best practices. Swedish Hospital has staff who participate in a variety of sub-groups via AHE including the following: AHE steering committee, food security/food access workgroup, mental health and substance use disorders committee and the CHNA committee. In addition to the above mentioned activities, Swedish Hospital regularly engages in conversation to advocate for community health improvement and support, including attending meetings with the local police districts on safety/community policing and meeting with organizations who serve disadvantaged populations within our community to develop relationships/partnerships to provide preventive health screenings (including mammograms), violence prevention resources, and other free health education offerings throughout the community. Schedule H, Part VI, Line 2 Needs assessment Endeavor Health screens adult patients in many care settings for social drivers of health (SDOH) to identify patients in need and connect them to resources. Collecting SDOH information deepens our understanding of the challenges patients face and guides navigation to critical services. EndeavorHealth screens for five key areas: food insecurity, housing instability, transportation barriers, interpersonal safety and utility difficulties. This data helps us understand patient needs and connect them to essential resources like food pantries. We have launched a Community Health Worker Program to address gaps in cancer screening rates, 30-day readmissions, and chronic disease outcomes, while also strengthening SDOH navigation and connections to community resources. Endeavor Health has a dedicated Community Impact and Engagement team made up of professionals across the system who are committed to strengthening our community's health and wellbeing. Areas of focus include economic improvement, affordable housing, health and safety, education, arts and culture and the environment. ETD is nonprofit mental health arts organization which focuses on collecting and sharing stories about mental health in an effort to disarm stigma through immersive multi-media arts installations.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistance Charity or discounted care is available for medically necessary services as defined by Medicare, to patients who meet the financial and documentation criteria defined below. Each situation is reviewed on an individual case-by-case basis. While not absolutely essential, the need for potential charity or discounted care should be established in advance of admission or rendering of service, or shortly thereafter. Notices in English, Arabic, Assyrian, German, Japanese, Korean, Mongolian, Polish, Russian, Spanish, and Vietnamese about the availability of Charity or Discounted Care are posted in the Emergency Room registration area, and the inpatient and outpatient registration areas. The information is also contained in the Patient Handbook, given to all patients upon inpatient admission. Patients and families are encouraged to speak to a financial counselor for more information and assistance, The Guidelines, along with the financial questionnaire, are posted in English, Arabic, Assyrian, German, Japanese, Korean, Mongolian, Polish, Russian, Spanish, and Vietnamese on the hospital's website. The Swedish Hospital billing statements describes the Financial Assistance program and necessary contact information. Our collection agencies and billing offices also coordinate charity care to potentially eligible patients. In order to be eligible for charity or discounted care, the patient must be willing to provide verification of income by filling out the Patient Financial Statement. The patient is responsible to voluntarily submit any and all documentation in order to be eligible to receive this discount. Swedish Hospital has 5 full-time financial counselors representing our hospitals, clinics, and physician offices. These counselors are responsible for helping patients understand their bills and coordinating financial assistance and insurance eligibility when appropriate. Any uninsured patient admitted to our hospitals or those identified at registration will have their case reviewed by a financial counselor. EHCO has also earned Certified Application Counselor Organization (CACO) status with the Centers for Medicare and Medicaid Services (CMS). Our counselors are certified and capable of helping patients navigate Medicaid and Insurance Exchange options. Swedish Hospital partners with Great Lakes Medicaid with the goal to provide under insured and uninsured patients with insurance coverage through various Medicaid programs. The service is offered to patients receiving care as an inpatient, in the emergency department and in various outpatient departments, and is tailored to guide the patient through a complex application process for applicable federal, state, and community benefit programs. During the registration and information gathering process, the financial counselors determine if the patient qualifies for medical assistance from other existing financial resources such as Medicare, Medicaid, Kid Care, Family Care or other state or federal programs, If the patient refuses to apply for existing financial resources or to provide information necessary to the application process, charity or discounted care cannot be granted. If the application for existing financial resources is denied, or has been previously denied, consideration for charity or discounted care will then be given. Once the Financial Assistance Application form is received, the Financial Service Center determines the eligibility for charity or discount care. SCH suspends the collection process while the Financial Assistance Application is being reviewed. The only criteria considered for Financial Assistance is income and family size. Income is evaluated against the matrix of Federal Poverty Guidelines to determine whether full or partial discount can be approved, Documentation of income is submitted in the form of paycheck stubs, income tax returns, Social Security checks, and/or other documents that are indicative of income. If the information submitted is not perceived to be accurate or reliable, SCH reserves the right to request additional documentation to substantiate income or family size. Patients who are approved for financial assistance and earn a family income less than 200% of the federal poverty guidelines will receive care at no cost for their patient balance after $150 per inpatient stay or outpatient services. The insured patient with a large balance due to deductibles and/or co-payments may be eligible for charity or discounted care. In order to qualify, the patient must complete the Patient Financial Statement and return it to the financial counselors for evaluation and recommendation. No legal action is taken against uninsured patients for the first one hundred twenty (120) days after discharge. In addition to the formal application process for Financial Assistance, Swedish Hospital may also grant Financial Assistance on a presumptive basis. Presumptive Financial Assistance approvals are based upon individual life circumstances such as enrollment in low-income government programs with income eligibility below 200 percent of federal poverty level income guidelines or verification of financial indigence through a credit rating agency and their calculated healthcare credit score. Patients who have a family income that is no more than 600% of the Federal Poverty Guidelines (as determined each year), and who do not have any health insurance (or coverage under workers' compensation, accident liability insurance, or other third party liability) as documented through SCH's insurance verification procedures, receive a discount in accordance with the Hospital Uninsured Patient Discount Act (210 ILCS 89/) (the Act).
Schedule H, Part VI, Line 4 Community information The Swedish Hospital service area is composed of 13 ZIP codes, with a total population of nearly 700,000. These ZIP codes encompass fourteen community areas in Chicago - Albany Park, Avondale, Edgewater, Forest Glen, Irving Park, Jefferson Park, Lake View, Lincoln Square, North Center, North Park, Portage Park, Rogers Park, Uptown, West Ridge - and the village of Lincolnwood. This community definition was determined because most of Swedish's patients originate from these areas. The Swedish service area population is projected to decrease slightly over the next five years. The highest utilizers of health care services are patients 65 and over, and this age group is expected to grow 11.7% over the next five years. The number of residents in age group 0-17 and 18-44 is expected to decrease 3.5% and 6.0% respectively, while age group 45-64 will increase 4.4%. The male population in the Swedish service area is slightly higher than the female population and is projected to decline slightly along with the female population over the next five years. The Swedish service area is primarily Non-Hispanic/Latino White (42%) but also has substantial Hispanic/Latino population (34%). The Hispanic/Latino population is projected to grow nearly 5% over the next five years, while Non-Hispanic/Latinos are projected to decrease 3% driven by Non-Hispanic/Latino White. There is a small growth projected in nearly all education levels within the service area over the next five years. "Greater than Bachelor's Degree" level is expected to slightly decline. The unemployment rate in the Swedish service area is 0.6% less than Illinois and 0.5% above Endeavor Health service area. Unemployment rates are projected to grow about 1% by 2027 in the Swedish service area, Endeavor Health service area and Illinois. The number of households in upper income brackets ($100,000+ annual income) is projected to increase while all lower income brackets are projected to decrease. The average household income in the Swedish service area is $110,620 while the median household income is $76,189. Household income levels in the Swedish area are less than Illinois for households making $100,000 or more but are projected to increase and exceed Illinois over the next five years. Note that 34% of households in the Swedish service area have a household income of less than $50,000 - compared to 25% within the Endeavor Health service area and 33% within Illinois overall. Residents within the Swedish service area living below 200% of the poverty level are greater than the Endeavor Health service area as well as Illinois overall. Additionally, some ZIP codes in Swedish's service area have as high as 42.3% of residents living below 200% of the poverty level. The percentage of uninsured residents in the Swedish service area is higher than both the Endeavor Health service area and the Illinois average and ranges from 5.4% to 16.8%, an indicator of the disparities that exist within the communities we serve. The percentage of residents with limited English proficiency in the Swedish service area is higher than both the Endeavor Health service area and the Illinois average and ranges from 2.8% to 18.4%, an indicator of the disparities that exist within the communities we serve. Schedule H, Part VI, Line 5 Promotion of community health The hospital furthers its exempt purpose by promoting the health of its community in many ways as follows: 1. A majority of the governing body of the hospital is comprised of persons who reside in the primary service area and are neither employees nor independent contractors of the organization, nor family members thereof. The organization extends medical staff privileges to all qualified physicians in its community for all departments. As a Not-For-Profit Organizations, the hospital reinvests earnings in the organization to maintain and enhance services that benefit the community served by the hospital. The organization develops and updates a strategic plan on a regular basis to identify needs and opportunities to deploy excess funds. Projects are evaluated based on organizational objectives and community needs, and are prioritized by senior management and the board of trustees. 2. The Endeavor Health Community Investment Fund is a dedicated resource aimed at fostering health and wellness, addressing social drivers of health (SDOH) and improving access to quality healthcare within our community. By strategically allocating these funds, we support local initiatives, partnerships and non-profit organizations that respond to priority community health needs. Whether it's funding for preventive health programs, grants for community health education or resources for mental health initiatives, our goal is to provide the supportive framework that helps community members thrive. During 2024, CIF partnerships in the communities served by the hospital included: - Between Friends - Expanded domestic violence support programs to end the cycle of abuse, including a 24-hour helpline and counseling services. Funds supported a bilingual counselor, financial empowerment workshops and wellbeing programs for domestic violence survivors and their families. - The Friendship Center - Supported food insecure populations through an enhanced facility, mobile outreach and connection to public benefits and resources in Chicago. The center is reaching capacity as need continues to drastically increase annually. In 2024, distributed more than 1.2M pounds of food to neighbors in need, which is nearly a 15% increase over the previous year and helped connect 700+ neighbors to SNAP benefits. - Lutheran Social Services of Illinois - Expanded access to behavioral health care at LSSI's Welcoming Center, a walk-in community mental health clinic that provides counseling, crisis intervention, psychiatric serves, and follow-up to hospitalizations. In 2024, expanded Welcoming Center hours to include Saturdays and Sundays that resulted in 200+ additional client encounters compared to previous year. - Howard Brown Health - Upskilled and expanded capacity to treat people experiencing complex trauma. In 2024, served 550+ new clients through behavioral health provider position. - Marjorie Kovler Center - Focused on improving mental health of youth through school-based trauma-informed services and capacity building for caregivers and staff. In 2024: 88 school personnel trained in trauma-informed services for youth and their families. - Midwest Refuah Health Center - Provided accessible, affordable, culturally sensitive primary, preventive and specialty medical, dental and behavioral healthcare to under-resourced, un/underinsured community members. In 2024: 2,800+ unique patients and 13,000+ total patient visits. Achieved FQHC Look-Alike status as of 12/12/2024. - ONE Northside - Engaged 187 participants in violence prevention programing under the age of 25 in addition to 45 Senn High School youth enrolling in the mentor program. 3. The System Office of Community Health Equity & Engagement (SOCHEE) is the governing body that provides thought leadership and shares best practices to inspire and drive equity and inclusion internally and externally. The hospitals senior executives are part of SOCHEE and help support this important work. The three key components of SOCHEE are Community Impact and Engagement (focused on our community), Health Equity (focused on our patients) and Diversity, Equity and Inclusion (focused on our team members). 4. Endeavor Health is a member of the Healthcare Anchor Network which catalyzes health systems individually and collectively to leverage their hiring, purchasing, investing, and other key institutional assets to create stronger local economies that sustain healthy communities. This intentional commitment creates partnership with the community for mutual benefit. 5. The hospital's competent and committed professional nursing staff provides their time and expertise to serve as clinical preceptors for nursing students who come to the organization to complete their clinical practicum requirements. The hospital remains committed to these efforts and strategic programs because they are inextricably linked to supporting the ever-growing need for nurses, which in turn improves access to healthcare for the patients and families served. The hospital is also a clinical rotation site for interns in many other healthcare areas such as radiology, lab, physical therapy, pharmacy and more. Clinical preceptors provide one-on-one teaching with student(s) assigned to them during the clinical practicum. The hospital operates an emergency department that is open 24 hours a day, seven days a week and serves all persons regardless of their ability to pay. 7. A standing Community & Mission Committee of the hospital board oversees and evaluates community outreach programs and services, as well as pastoral care and chaplaincy offerings. The committee reviews the community health needs assessment and ensures the implementation plan addresses identified needs. The committee, comprised of board members, senior executives, physicians and staff, provides oversight in monitoring and evaluating the effectiveness of strategies and initiatives which serve and support the community, especially those who are under-resourced.

Schedule J, Part I, Line 3 Arrangement used to establish the top management official's compensation A detailed compensation review of the top executives, including the Chief Executive Officer, is conducted annually. Market data is collected and assessed by an external independent compensation consultant who specializes in compensation consulting within the healthcare industry. The work product from this study is reviewed separately with Endeavor Health legal counsel. Market data for base and variable compensation is assessed annually for integrated delivery systems and academic medical centers that are similar in size and complexity. The market assessment includes assessing job content in order to make appropriate market data comparisons. Specific recommendations are then reviewed, discussed and approved as appropriate with the Endeavor Health Compensation Committee, in session with legal counsel present, in advance of implementation.

Form 990, Part III, Line 1 Organization's Mission Swedish Covenant Health (Swedish Hospital) follows the mission of Endeavor Health. The mission of Endeavor Health is to "help everyone in our communities be their best." Central to this mission is a commitment to providing clinical programs and services that meet community health needs, while also pursuing continuous improvement to identify and understand future needs.
Form 990, Part III, Line 4 Program Service Description Endeavor Health Swedish Hospital, which opened in 1886, serves the culturally-diverse residents of Chicago's north and northwest side communities as a safety-net hospital, with a full-service hospital campus located in the Lincoln Square neighborhood. Swedish Hospital offers advanced clinical care in more than 50 medical specialties, which includes the Jack and Mayora Rosenberg Women's Health Center, offering 3-D mammography, and their Joint Commission-certified Primary Stroke Center. Swedish provides a full range of comprehensive health and wellness services offering 150+ wellness programs annually, and Chicago's only certified medical fitness center, Galter Life Center, and is one of six regional leads for Healthy Chicago Equity Zones. During 2024, Swedish Hospital total admissions were 10,841 and total inpatient days were 39,715. Physical Medicine/Rehab Services - Swedish Hospital is dedicated to providing a comprehensive Rehab Unit that combines the disciplines of physical therapy, speech therapy, occupational therapy and social work to help patients regain their strength and skills. The Rehab unit features a special room that simulates a home environment so patients can practice with home-like carpeting, furniture and fixtures while supervised by nurses. During 2024, 189,462 inpatient and outpatient treatments for rehab, speech and occupational therapy were provided for a total cost of $7,017,291. Cardiac Care and Rehab - The Cardiac Rehabilitation program offered by Swedish Hospital has improved the lives of many individuals with such innovative programs as "coronary Heart Disease Reversal." Swedish Hospital also offers a state-of-the-art Cardiac Catheterization Lab, and other diagnostic and preventive programs. During 2024, Swedish Hospital performed 2,788 procedures in the Cardiac Catheterization Lab for a total cost of $13,174,876. In addition to the program services listed above, Swedish Hospital also offers health screenings, health promotions, education, and prevention services. Swedish Hospital offers year-round health screenings to members of the community. Blood pressure screening events are provided in collaboration with the hospital's Stroke Education Coordinator. Each year free or reduced-cost cancer screenings are offered to the community. These included prostate, skin, cervical and breast screenings. Additionally, dozens of programs led by physicians, nurses, dietitians, personal trainers and other clinicians and experts from the hospital address topics of cancer prevention, treatment, nutrition, fitness and other healthy lifestyle recommendations. During 2024, Swedish Hospital hosted 79 programs at the Community Wellness Center on the hospital campus, with a total of 148 sessions. Additionally, Swedish Hospital participated in dozens of community outreach events in collaboration with community partners to serve thousands of local community members. Programs included health fairs, health education seminars, screenings and other health related activities. Community Wellness and Prevention programs continue to serve the community with more than 100 events held at the Hospital or in the community through collaboration with community partners. These events include health fairs, screenings, daytime and evening lectures, seminars, classes, Speakers' Bureau presentations, children's events and other health related activities. Throughout the year the Hospital collaborated with dozens of community organizations to provide wellness and prevention programs. Medical Education Swedish Hospital is a teaching hospital and part of Endeavor Health, with residency programs in emergency medicine, family medicine, internal medicine, and podiatric medicine and surgery. Swedish Hospital also has fellowship programs in critical care medicine and pulmonary critical care medicine. Swedish Hospital utilizes board-certified physicians who are either independent contractors or are employed by Endeavor Medical Group, to provide teaching, training and supervision of the residents in Swedish Hospital's Family Medicine (17 residents), Internal Medicine (30 residents), Podiatric Medicine and Surgery (6 residents) residency programs as well as the Critical Care (3 fellows) and Pulmonary Critical Care Fellowships (3 fellows). Swedish Hospital has an affiliation agreement with McGaw, the postgraduate arm of Northwestern University, to be a training site for their family medicine residents. Surgical residents from the Ascension St Joseph's general surgery residency program also rotate at Swedish Hospital. OBGYN residents from UIC also rotate at Swedish Hospital. Swedish Hospital is also a training site for students of three medical schools; Chicago College of Osteopathic Medicine (33), and Arizona College of Osteopathic Medicine (15), both part of Midwestern University, and for the University of Nicosia (40).

Form 990, Part VI, Line 11b Review of form 990 by governing body The Form 990 was reviewed by executive management and an outside accounting firm. The Form 990 was then provided to the Endeavor Health's Board of Trustees for review for the ability to ask questions of management prior to filing.
Form 990, Part VI, Line 12c Conflict of interest policy All officers, directors and employees are required to report potential conflicts of interest to the Compliance Officer when his/her circumstances could create a conflict of interest, or prior to their arising, so that the health system can proactively review the report to identify actual and potential Conflicts of Interest. In addition, on an annual basis, members of the Board of Directors, Corporate Officers, and other key employees will be provided with a Conflict of Interest Questionnaire, which is used for purposes of reporting potential Conflicts of Interest. Subsequent to reporting, and depending on the nature of the matter, the Compliance Officer will review the reported information and arrive at a determination regarding the matter based upon his/her knowledge of the organization and/or in consultation with other members of management. Determinations will be reviewed with the Executive Leadership Team or designee for members of management, all categories of physicians, and the Board of Directors. Determinations will be reviewed with the Board of Directors for senior management and members of the Board of Directors. If it is determined that a Conflict of Interest exists, appropriate mitigating or remedial measures may be taken through a management plan. If a management plan has yet to be developed and the individual is involved in discussion related to his/her conflict, the individual must disclose the Conflict of Interest to those involved in the conversation and must recuse him/herself from participating in the conversation and making a decision on behalf of the health system. If this conversation takes place at a Board meeting, the minutes of the meeting should reflect the fact that the Conflict of Interest has been disclosed and the individual has recused him/herself. If the issue or circumstances cannot be adequately addressed through a management plan or if the proposed or actual arrangement is inconsistent with the health system's Guiding Principles for Conflicts of Interest, the conflict will be eliminated. Disclosure of the management plan may be made to appropriate individuals or committees, which may include patients, students, a department, group, or others as necessary.
