UF Health Jacksonville, part of UF Health, provides a broad range of services supporting patient care, education, and community health across northeast Florida and southeast Georgia. Its primary facility, UF Health Jacksonville, is an academic medical center and the Jacksonville campus of the University of Florida Academic Health Center, offering advanced specialty care and serving as a major training site for medical students, residents, and other health professionals. Additional programs include UF Health North, a community-based campus with a full-service hospital, emergency care, outpatient surgery, and diagnostic services; a network of primary and specialty care practices staffed by UF faculty physicians; and key regional services such as TraumaOne, the area's only Level I trauma center, a Level III neonatal intensive care unit, and the UF Health Proton Therapy Institute for advanced cancer treatment. Together, these programs provide an integrated continuum of care while advancing UF Health's mission to promote health through outstanding and high-quality patient care.

Form and Line Reference Explanation
Schedule H, Part V, Section B, Line 3E The significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA. The information gathered through key stakeholder interviews, group listening sessions, and intercept surveys was collected and organized into twelve general categories. This organized data set was evaluated quantitatively through calculations such as the percentage of individuals providing input who identified a given issue as a health need in the community and the frequency of that issue being ranked by those providing input as one of the community's top three needs. Publicly-available data sets, including US Census Bureau statistics, were incorporated to supplement this analysis. This data was presented to health system representatives and community members for discussion during a prioritization workshop hosted by the Jacksonville Nonprofit Hospital Partnership (the Partnership) on June 18, 2024, resulting in four health needs categories emerging as the community's most pressing needs: Access to care, mental health, food environment, and housing.
Schedule H, Part V, Section B, Line 5 Facility A, 1 Facility A, 1 - SHANDS JACKSONVILLE MEDICAL CENTER, INC.. Between March and April of 2024, primary data was collected from approximately 200 individuals via the following methods: (1) Interviews conducted with key stakeholders. Sixteen key stakeholder interviews were conducted during this assessment. During these 60-minute interviews, key stakeholders shared their greatest concerns around health needs, health equity, and social determinants of health for those they serve. (2) Group listening sessions. Fifteen group listening sessions were conducted, with a collective 109 individuals participating in these sessions. These 60-minute facilitated discussions were conducted with local service providers to gather information about participants' experiences and observations working with vulnerable populations. (3) Intercept surveys. Seventy-two intercept surveys were conducted to gather input through structured conversations with community members experiencing health needs, each lasting 15-20 minutes. The 2024 Community Health Needs Assessment was conducted with the other hospitals comprising the Jacksonville Nonprofit Hospital Partnership, a network of five nonprofit health systems serving Northeast Florida. "SJMC".
Schedule H, Part V, Section B, Line 11 Facility A, 1 Facility A, 1 - SHANDS JACKSONVILLE MEDICAL CENTER, INC.. The significant health needs identified in the 2024 CHNA were: 1. Access to care 2. Mental health 3. Food environment 4. Housing Access to care: SJMC operates the only Level I Trauma Center in northeast Florida. In November 2024, SJMC broke ground to expand its trauma center, with the first phase of this expansion increasing the number of emergency and trauma rooms from 78 to 125. Plans for future phases of this expansion include renovations of the older components of the facility, along with new diagnostic equipment, a new radiology section, and dedicated behavioral health space. SJMC expanded access to care for stroke patients in two ways during the year. First, a new telehealth service went live in January 2025 to allow for quicker identification and treatment of individuals who may be experiencing a stroke. This new service is available 24 hours a day, 7 days a week. Then, in May 2025, SJMC unveiled its first Mobile Stroke Treatment Unit ("MSTU"), a technology that allows stroke patients to be assessed by medical professionals sooner than would be possible in cases where the patient needs to be transported to a hospital first. Together with standard emergency response services, the MSTU is dispatched to the location of 911 callers reporting a stroke. Upon reaching the patient, the MSTU performs a CT scan on the patient that is reviewed by a stroke neurologist while the patient is en route to one of Jacksonville's stroke centers, and treatment can begin immediately upon confirming that the patient suffered a stroke. Deployment of the MSTU is estimated to reduce time to treatment by an average of 32 minutes. Mental health: Between the year ended June 30, 2025, and the two preceding fiscal years, SJMC has focused on addressing education and resources surrounding access to lethal means and safety within the home. Through these efforts, SJMC provided counseling services to 2,015 individuals and families, each of whom received a lock bag for medication, and trained 83 additional healthcare providers in this specialty. Food environment: SJMC operates the Food Pharmacy Program, which provides access to fresh fruits, vegetables, whole grains, and lean proteins for food-insecure community members with chronic conditions such as diabetes, hypertension, and obesity. Thus far, more than 400 individuals have benefited from the Food Pharmacy Program. SJMC has also partnered with Blue Zones Project to address access to healthy foods in the community. Working collaboratively with community grocery stores and restaurants to establish a tailored approach to help each of these establishments implement and maintain evidence-based practices, Blue Zones Project has developed criteria that grocery stores and restaurants must meet in order to become "Blue Zones Approved," of which all are centered around ensuring that healthy choices are easily accessible. To date, seven restaurants and eight grocery stores in the Jacksonville area are Blue Zones Approved. Needs SJMC Will Not Address No entity can address all of the health needs present in its community. SJMC is committed to serving the community by adhering to its mission, using its skills and capabilities, and remaining strong so that it can continue to provide a wide range of community benefits. SJMC does not have specific plans to address the following health priority that was identified in the 2024 Community Health Needs Assessment. Housing: SJMC will not address housing due to limited resources available and the fact that other community organizations are currently addressing the community's housing needs, including United Way of Northeast Florida, LiftJax, LISC, Blue Zones Project Jacksonville, and The Sulzbacher Center.

Schedule H, Part VI, Line 2 Needs assessment SJMC's community benefit activities are a planned, managed, organized, and measured approach to meeting identified community health needs. Through collaboration with the community and other partners, SJMC is committed to improving the health and well-being of the communities it serves through ongoing efforts to monitor health needs and respond accordingly. SJMC's Patient and Family Partnership Council for Quality and Safety represents the community by engaging with UF Health Jacksonville leaders to provide insight into care experiences, identify opportunities for improving services, brainstorm potential patient-centered solutions, and integrate the voice of patients and families into the daily work of professional caregivers. Stakeholder engagement is a key component of SJMC's efforts to promote health equity. SJMC collaborates with a variety of partners, including The University of Florida's Urban Health Alliance (UHA), to identify and implement programs that address social determinants of health using community-initiated strategies to address problems unique to Jacksonville. This collaboration hosts events that are designed to engage community members, with nearly 5000 surveys completed through more than 100 outreach events since 2020.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistance The eligibility for Financial Assistance under federal, state or local government programs is determined under the organization's Financial Assistance Program. Ideally, eligibility is obtained upon admission or before discharge. The organization provides notice of the Financial Assistance Program via the patient's billing statements. Information about the organization's Financial Assistance Program is also available on the organization's website. Education is also provided through distribution of the plain language summary, hospital staff, signage throughout the hospital and referrals from community organizations.
Schedule H, Part VI, Line 4 Community information SJMC serves the northeast Florida counties of Duval and Nassau, which collectively encompass 73 zip codes. Located in northeastern Florida, this predominantly urban area includes Jacksonville, which is the largest city by landmass in the continental United States. SJMC consists of two hospital locations, UF Health Jacksonville and UF Health North, that are approximately 10 miles away from each other and operate under a single hospital license. The majority of UF Health Jacksonville patients come from Duval County, while UF Health North patients are more representative of both Duval and Nassau Counties. Of the approximately 1.13 million people who reside within the 1,551 square miles of Duval and Nassau Counties, 48.7% are male and 51.3% are female. Approximately 57.4% of this population is white, with the Black/African American population accounting for 27.2%, Asian 4.4%, multiple races 7.5%, and all others at 3. 5%. 10.5% of the population is of Hispanic or Latino ethnicity. The median age of Nassau County residents is 46.3, which is nearly a decade older than Duval County's 36.6 median age. This difference could be attributable to the fact that nearly 23% of Nassau County is age 65 or older, as compared to the 14.7% of Duval County in this age group. 17.42% of Nassau County residents live with a disability, while 12.98% of Duval County residents live with a disability. The median household income in Nassau County ($84,085) is significantly higher than that of Duval County ($65,579), which is consistent with the differences between these two counties in terms of their respective poverty rates (9.21% in Nassau County and 14.34% in Duval County) and their respective percentage of households receiving SNAP benefits (8.2% in Nassau County and 14.95% in Duval County.) In addition to UF Health Jacksonville and UF Health North, there are nine other acute care facilities licensed as hospitals in Duval and Nassau Counties. Five federally designated medically underserved areas and one federally designated medically underserved population are present in the community.
Schedule H, Part VI, Line 5 Promotion of community health A majority of SJMC's governing body is comprised of persons who reside in the organization's primary service area who are neither employees nor independent contractors of the organization, nor family members thereof. SJMC extends medical staff privileges to all qualified physicians in its community. SJMC reinvests surplus funds to invest in patient care needs into capital improvements, expansion of clinical capacity, advanced diagnostic and treatment technologies, medical education and residency programs, research initiatives, and community health programs. Recent reinvestments include trauma center expansion, advanced stroke treatment capabilities, behavioral health infrastructure, and technology upgrades that enhance patient care delivery. These reinvestments ensure that resources are used to strengthen services and improve health outcomes within the communities served.

Schedule I, Part I, Line 2 Procedures for monitoring use of grant funds Funds included in the grant paid to University of Florida Jacksonville Physicians, Inc. are monitored to ensure funds are spent in accordance with the existing written agreement. The amount is based on negotiations between in-house counsel, VP of Finance and/or CFOs, President, Dean and/or CEO of each organization. If spending patterns change during the fiscal year, amounts can be reviewed and amended. Funds paid to WOLFSON CHILDREN'S HOSPITAL are agreed upon in the written agreement. SJMC monitors the funding and ensures that proper allocation of funds are paid subsequent to each fiscal year based on financial performance. Funds paid to Shands Jacksonville Foundation, a related organization, are monitored throughout the year to ensure the funds are spent in accordance with the written agreement.

Form 990, Part VI, Line 15 PROCESS TO ESTABLISH EXECUTIVE COMPENSATION Compensation of the organization's officers is reviewed and approved by UF Health Corporation, a related organization; therefore, Form 990 Part VI Lines 15a and 15b have been answered "no" in accordance with the Form 990 instructions. Pursuant to the organization's bylaws, the UF Health Corporation Board of Directors has established a compensation committee comprised of individuals serving on the Board of Directors. The compensation committee is responsible for reviewing and approving executive compensation, after consultation with the CEO of UF Health Corporation. Officer compensation is subject to the University Governance Standards and any Board of Directors Conflicts of Interest policies. The compensation committee of UF Health Corporation engages Mercer, an independent consultant with expertise in the development of compensation surveys on executive pay levels across different industries, to provide competitive data and guidance on determining appropriate and reasonable ranges and salaries. Mercer annually reviews all aspects of executive compensation for senior UF Health officers and key employees - including all elements of supplemental benefits - to ensure that total remuneration is reasonable. The compensation committee of UF Health Corporation utilizes the information provided by Mercer when reviewing and approving executive compensation. This process is undertaken annually and was last conducted during fiscal year 2025. The compensation committee documents deliberations and determinations in committee meeting minutes.
Form 990, Part VI, Line 1a Delegate broad authority to a committee The Executive Committee shall consist of the following members: 1. The CEO of the University of Florida Health Corporation ("UF Health Corp"); 2. The Chairperson of the organization's Board of Directors; and 3. Any other Directors as determined by the Chairperson after consultation with the Board of Directors. Subject to the limitations contained in the organization's Restated Articles of Incorporation and University Governance Standards, the Executive Committee shall have such powers and authority as the Board of Directors may delegate to it, including but not limited to the power to act on behalf of the Board. The Executive Committee may consider matters that are within the scope of other committees at the discretion of the Chairperson, the CEO of UF Health Corp, or the Board of Directors. Minutes of all Executive Committee meetings shall be recorded and forwarded to the Board of Directors.
