Explanation:

I believed my organization's return had already been filed by the due date. I later discovered it had not been submitted due to an unintentional oversight. As soon as I became aware of the error, I promptly filed the return and have implemented procedures to verify future filings. I respectfully request consideration for penalty relief based on reasonable cause.
