| Creating Health Access for Next Generation Equity (CHANGE) | Part XIV, Line 2c | Creating Health Access for Next Generation Equity (CHANGE) The AstraZeneca Foundation (Foundation) is committed to ensuring that everyone in the US has a fair and just opportunity to attain their highest level of health. The Foundation works to accomplish this by supporting nonprofit organizations with initiatives that address social determinants of health, or the conditions in which people are born, grow, live, work and age that impact health.As an important determinant of health outcomes, a key priority for the Foundation is improving access to timely and quality healthcare services. The Foundation recognizes that access to quality healthcare can be challenging due to barriers such as a lack of health insurance, transportation, employment and general socioeconomic status, among other factors. Accordingly, the AstraZeneca Foundation has established the Creating Health Access for Next Generation Equity (CHANGE) program. The CHANGE program awards Foundation grants to U.S.-based, nonprofit 501(c)(3) organizations or similar nonprofit organizations working in innovative, collaborative ways at the community level to improve access to quality healthcare. Our aim is for funded organizations to maximize the benefit to community members and community-based organizations and to work toward continuing their services beyond potential AstraZeneca Foundation funding.The intent of the 2025 grants is to provide continued funding for general operating support to organizations that received CHANGE grants in 2023 and 2024 to continue to enhance access to quality healthcare and to share lessons learned with other organizations to help expand the impact of their efforts. Organizational Criteria Invited organizations need to maintain their status as a U.S.-based, nonprofit organization with a 501(c) designation or a public school, government entity or municipal institution that is eligible to accept tax-deductible, charitable contributions. Organizations that are a 501(c)(3) and also have an IRS designation as a 509(a)(3) are ineligible for funding. After submitting the application, organizations cannot make substantive changes to their tax status. Once selected for a grant award, organizations cannot reassign, transfer or credit their grant to another entity, including any related entity, without prior written approval by the Foundation. Any changes in an organization's tax status after submitting the application or being selected for a grant award must be reported immediately and may impact the organization's eligibility to receive the grant. The organization must continue to be based in the United States or its territories and serve communities in at least one of the following locations: Delaware, Maryland, Pennsylvania or Washington DC.Funding Criteria The Foundation will fund requests between $200,000 and $250,000 for the third year. There is no guarantee of funding. o To be considered for continued funding, organizations must meet or exceed objectives overall in their Year Two Mid-Year Evaluation, as assessed by the Foundation's evaluation partners. Organizations not meeting or partially meeting objectives overall will be considered on a case-by-case basis by the Board of Trustees. Organizations that are applying for continued funding must complete and submit a brief grant application confirming goals and activities for the grant year and reaffirming the organization's representations and warranties and other obligations under the grant agreement. Any budget expenses allocated for healthcare provider compensation may support a portion of or up to the full annual salary of the healthcare provider but cannot be in addition to the healthcare provider's annual salary for the grant timeframe paid for by the applying organization. While indirect costs and fringe benefits are not capped, grant requests should demonstrate that most funding will directly benefit community members and community-based organizations.Priority Communities Organizations must continue to demonstrate a commitment to improving equitable access to quality healthcare for all patients. Organizations that provide services in areas of low-income and/or remote/rural areas are expected to continue to do so.Focus Areas/Services Organizations must continue to provide services tailored to the needs of their communities to help improve access to quality healthcare while considering the social determinants of health that impact community members. This may include but is not limited to: o Providing access to care services (e.g., screenings, wellness and prevention services) and coordination of care (e.g., case managers, health coaches, promotores) o Mitigating barriers that impact access to care services (e.g., transportation, affordability physical accessibility, translation services, plain language materials) Organizations are expected to continue to: o Offer innovative, community-engaged approaches to improving access to quality healthcare o Include collaboration with other community-based organizations in their approach o Create a new partnership and/or coalition or expand existing partnerships with other institutions to help breakdown pre-existing silos and maximize benefit to the community through collaboration o Engage with their local communities/populations served to help guide decisions around services and accelerate local change o Conduct community-based outreach o Demonstrate robust plans to sustain their access to quality healthcare efforts (via fundraisers, grants, partnerships, reimbursable services etc.) and are encouraged to show potential to replicate their approach Organizations that include a focus on promoting screenings, early detection, treatment and/or continuity of care for cardiovascular, renal, respiratory or immunologic diseases and/or cancers are expected to continue to do so. Organizations are encouraged to measure their overall impact on community health. Organizations need to develop and disseminate one of the following items during the grant year to share best practices, impacts and lessons learned from their funded approach with organizations who may be interested in replicating aspects of their approach: o conference presentation o webinar/presentation o toolkit o white paper o video o publication in a scholarly journal o Other dissemination strategies may be considered.Exclusions The Foundation does NOT fund applications that focus exclusively on the following: o Capital investments and unsolicited capital campaigns o Medical education and/or training (i.e., training necessary to obtain a degree or license) for healthcare professionals that is more than incidental and that is the primary focus of the grant's activities o Research or clinical trials o Healthcare provider salaries o Faith-based programs that are NOT open to the community-at-large The Foundation does NOT support: o Initiatives outside the United States or its territories o Individuals o For-profit organizations o Endowments o Journals or advertising o Political causes, lobbying, fraternal or social organizations o Religious organizations whose activities are not open to the general public o Nonprofit organizations that discriminate on the basis of age, race, ethnicity, religion, national origin, gender, sexual orientation, marital status, military service, veteran status or disabilityGrant Application Applying organizations will need to: o Provide 1) a copy of their current IRS 990 form or documents that confirm eligibility to receive charitable donations and 2) audited financials for the past fiscal year. (If audited financials are not available, unaudited financial statements may be submitted). o Identify a goal for the number of individuals to be reached through their access to quality healthcare efforts. o Identify key short-term goals of their efforts to improve access to quality healthcare during the grant year and continue to measure the long-term impact of their efforts across the three years of CHANGE funding.Grant Execution Grant awardees will need to submit regular reports and other requirements to the Foundation according to deadlines and processes set forth in the Letter of Agreement Addendum. Written approval from the Foundation is needed from grant awardees prior to making a major change in objectives, budget or scope. Some examples include but are not limited to service location, target audience and goals. Grant awardees (1-2 individuals per organization) need to be available to participate in a Community Advisory Board, including up to monthly virtual meetings (with some potential in-person opportunities) with fellow grant awardees, Foundation staff and its evaluation partners for capacity-building activities based on identified needs and to refine the reporting and evaluation plan as needed. |
| Part XIV, Line 2d | The AstraZeneca Foundation's mission is to advance health equity and foster community wellbeing. Through its signature program, Creating Health Access for Next Generation Equity (CHANGE), the Foundation provides grants to US-based nonprofit organizations working to improve access to quality healthcare in their communities. As part of their efforts, organizations focus on promoting screenings, early diagnosis, treatment and/or continuity of care for cardiovascular, renal, respiratory or immunologic diseases and/or cancers as well as expanding partnerships with the intent of breaking down institutional silos and maximizing benefit to the community through collaboration. CHANGE is more than a grantmaking platform; it is a philanthropic collaborative by which the Foundation brings funded organizations together for meaningful dialogue, capacity-building and shared learning and implementation.Since 2023, funded organizations have collectively reached more than 59,000 individuals with direct services, events and programming and have improved healthcare access through addressing barriers to care, expanding service offerings and reach, and/or integrating patient-centered practices. In the second half of 2025, six current awardees serving communities in Delaware, Maryland, Pennsylvania and Washington, D.C., received third year CHANGE grants, totaling $1.46M, to continue their work to improve access to quality healthcare and to share lessons learned.
