SERVICES ARE DELIVERED BY LICENSED AND CREDENTIALED CLINICIANS USING EVIDENCE-BASED PRACTICES AND ARE AVAILABLE AT LOW OR NO COST THROUGH GRANT FUNDING, MEDICAID, AND SLIDING FEE SCALES. THESE SERVICES SUPPORT STABILIZATION, RECOVERY, AND IMPROVED BEHAVIORAL HEALTH OUTCOMES FOR INDIVIDUALS AND FAMILIES. CFPC'S RECOVERY SUPPORT DEPARTMENT PROVIDES PEER-BASED RECOVERY SERVICES, CASE MANAGEMENT, RECOVERY PLANNING, AND LINKAGE TO TREATMENT, HOUSING, AND COMMUNITY RESOURCES FOR INDIVIDUALS AND FAMILIES IMPACTED BY SUBSTANCE USE DISORDERS. SERVICES ARE PROVIDED IN PERSON AND VIRTUALLY AND ARE DESIGNED TO SUPPORT INDIVIDUALS ACROSS ALL STAGES OF RECOVERY. CFPC ALSO PROVIDES SPECIALIZED OUTREACH, INCLUDING ENGAGEMENT WITH INDIVIDUALS IN EMERGENCY DEPARTMENTS FOLLOWING OVERDOSE, AND SUPPORTS FAMILIES AND CAREGIVERS THROUGH RECOVERY NAVIGATION SERVICES. RECOVERY SUPPORT SERVICES ARE INTEGRATED INTO A CONTINUUM OF CARE TO PROMOTE LONG-TERM RECOVERY, REDUCE BARRIERS TO SERVICES, AND IMPROVE OVERALL STABILITY. CFPC ALSO OFFERS HARM REDUCTION SERVICES THAT REDUCE THE NEGATIVE HEALTH IMPACTS ASSOCIATED WITH SUBSTANCE USE AND INCREASE ENGAGEMENT WITH CARE. HARM REDUCTION SERVICES INCLUDE OVERDOSE PREVENTION EDUCATION, NALOXONE DISTRIBUTION, AND ACCESS TO HARM REDUCTION SUPPLIES. THESE SERVICES SERVE AS AN ENTRY POINT TO CARE FOR INDIVIDUALS NOT YET READY OR ABLE TO ENGAGE IN TREATMENT AND SUPPORT CONNECTION TO RECOVERY AND SUPPORT SERVICES. IN ADDITION TO DIRECT SERVICES, CFPC ADVOCATES FOR POLICIES THAT PROMOTE WELLNESS AND INCREASED ACCESS TO SERVICES AND FUNDING FOR INDIVIDUALS WITH SUBSTANCE USE DISORDERS.
