	The Utility provides a public water system to its 5,103 residential agricultural commercial and industrial members. The Utility uses Green River as a water source and produces water at its treatment plant located on Clean Water Drive in Morgantown Kentucky. The Utility pumps finished water from the treatment plant through 519 miles of distribution main and 33 miles of service line to the metering point of each member of the Utility.

Part VI, Line 3	The Utility has a contractual agreement where operational and daily managerial duties are performed by Warren County Water District. The Utility participates in a Joint Operations Agreement with Warren County Water District and Simpson County Water District. Both Warren and Simpson are divisions of county government. Under the agreement each water utility pays the actual cost of labor equipment and other expenses for services provided by any other water utility on its behalf. The utilities share the same General Manager officer; however each utility is independently governed by its own board of directors and each utility provides water service to customers within its respective county as approved by the Public Service Commission of Kentucky.
Part VI, Line 6	Yes the Utility has members who make application for and receive water service as a customer of the Utility in accordance with the Rates Rules and Regulations Tariff approved by the Public Service Commission of Kentucky. The Utility will install maintain and operate a water distribution line from the source of the water supply to the metering point of each member of the utility.
Part VI, Line 7a	The Utility had 5,103 customers who are members entitled to one vote each at meetings to elect the governing board of directors.
Part VI, Line 11b	A draft of Form 990 is completed by the Manager of Finance & Administration who will consult with the Utilitys certified public accounting firm on any content that requires clarification. A final draft is reviewed by the General Manager and the approved Form 990 is presented to the Board of Directors at the next scheduled board meeting.
Part VI, Line 12c	Bids and proposals received through the Utilitys procurement process and payments made to vendors are reviewed to insure against conflict of interest. Annually the Utilitys audit firm conducts an inquiry of personnel to identify any fraud or conflict of interes
Part VI, Line 19	The Utilitys conflict of interest policy and governing documents are available upon request at its office. The financial statements and audit report are available to the public through the Public Service Commission of Kentuckys website psc.ky.gov. In addition the Utilitys financial statements and audit report are available upon request at our office.
