| CORE FORM, PART III, STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | Key FY 25 Highlights: In FY 25, Star Community Health (SCH) continued to strengthen access to care and broaden its range of services. Behavioral Health was expanded at Sigal KidsCare, Sigal Women's Health, Bethlehem Family Medicine and Bethlehem Internal Medicine. Notably, a bilingual licensed professional counselor (LPC) joined the team at Sigal KidsCare, ensuring patients and families can receive vital mental health support in the language they feel most comfortable. Additionally, SCH successfully completed its scheduled Operational Site Visit (OSV) by the Health Resources and Services Administration (HRSA). Quality Achievements: In FY 25, Star Community Health showed significant progress in quality performance and preventive care. SCH continues to have Patient-Centered Medical Home (PCMH) recognition for all primary care and pediatric practices. SCH improved already high-performing delivery of care in HIV screening, documentation of childhood and adult healthy weight metrics, placement of dental sealants and control of hypertension. SCH practices have half the number of patients with poorly controlled diabetes compared to the national and state average for FQHCs and continue to be in the top national quartile among FQHCs for cancer screenings (cervical, breast, colorectal), immunization delivery and depression screenings. Star Community Health continued to prioritize diabetic eye health through comprehensive screening efforts. A total of 792 screenings were completed during the year, contributing to 3,566 screenings overall since program inception. Of the 2025 screenings, 223 (28.2%) revealed pathology and 146 (18.4%) were identified with diabetic retinopathy. Through the IRIS program, 155 cases (19.6%) were successfully saved, and 669 gradable orders were processed. These results underscore our commitment to early detection and intervention, helping prevent vision loss and improving quality of life for patients living with diabetes. Star Community Health earned American Heart Association (AHA) recognition with Gold-level badges for Diabetes and Cholesterol management under the "Know Your Numbers" initiative and a Silver Plus badge for Hypertension control. SCH launched a medication dispensary program at our largest site that will help provide free medication to eligible uninsured patients and continue to offer personalized assistance with medication through discount initiatives and enrollment in assistance programs. These achievements reflect SCH's ongoing commitment to delivering high-quality, patient-centered care and advancing health outcomes across our communities. Grants FY 25 ------------ 1) Title V Maternal and Child Services Grant (year five of grant) - Amount received: $50,000 - Allocation: $50,000 - Population Served: To provide access to quality health care for children and youth with special health care needs (CYSHCN) and their families with low incomes and/or limited availability of care. 2) Congressionally Directed Spending - Construction Projects - Sponsor: Senator John Fetterman - Purpose: Replacement of Allentown Dental Van - Amount Awarded: $500,000 - FY 25 Allocation: $152,124 - Population Served: Students and families within the Allentown School District through mobile dental visits 3) County of Northampton Department of Community and Economic Development - Purpose: Replacement of Bethlehem Dental Van - Amount Awarded: $600,000 - FY 25 Allocation: $152,124 - Population Served: Students and families within the Bethlehem Area School District through mobile dental visits Community Engagement: Star Community Health demonstrated a strong commitment to community engagement and outreach throughout FY 25, investing $43,725 in sponsorships and direct services to support local organizations dedicated to improving the lives of community members. Among the most impactful initiatives was a partnership with St. Luke's Cancer Education Program and Norwescap's Cancer Education and Early Detection Program to host the first free mammogram event at SLUHN Warren Campus in New Jersey. This event provided 21 women with free mammograms and connected them to primary care and OB/GYN services at SCH. Building on its success, a second event was held at the Easton Campus, serving 18 women, and a third event is scheduled for FY 26 in Lehighton. Star Community Health continued its annual scholarship program, awarding five $1,000 scholarships to high school seniors within its service area and offering recipients the opportunity to shadow providers and SCH leadership during the summer. Star Community Health provided medical and dental care to more than 50 migrant workers at Jaindl Turkey Farm through its mobile health and dental units, ensuring that the workers were healthy and had access to important medical and dental care during their time at the farm. Star Community Health actively supported numerous nonprofits and community partners during National Health Center Week and the holiday season, including preparing meals at Safe Harbor in Easton, assisting with the Bethlehem YMCA Holiday Bash and collecting monetary and in-kind donations for local schools and food pantries. These efforts reflect Star Community Health's ongoing mission to improve health outcomes and strengthen community connections. |
