IN FY 2025, 196,778 OLDER ADULTS AND CAREGIVERS WERE SERVED THROUGH THE WIDE RANGE OF TITLE III AND OTHER RELATED PROGRAMS INCLUDING HOME DELIVERED MEALS, MEAL KITS, AND CONGREGATE DINING, LONG TERM CARE OMBUDSMAN, ADULT PROTECTIVE SERVICES, LEGAL SERVICES, HOME PRESERVATION, PREVENTION OF SOCIAL ISOLATION AND THE OTHER SUPPORTIVE SERVICES.

HOME OPTIONS PATH TO EMPOWERMENT (H.O.P.E.) IS FUNDED BY THE ILLINOIS DEPARTMENT OF HUMAN SERVICES UNDER THE COLBERT CONSENT DECREE. H.O.P.E. ASSISTS COOK COUNTY NURSING FACILITY RESIDENTS, 18 AND OLDER AND ON MEDICAID, TO TRANSITION BACK INTO THE COMMUNITY AND LIVE AS INDEPENDENTLY AS ABLE. H.O.P.E. CLIENTS WORK WITH OUR SKILLED PROFESSIONALS, INCLUDING SOCIAL WORKERS, OCCUPATIONAL THERAPISTS, NURSES, LICENSED CLINICAL PROFESSIONAL COUNSELORS AND OTHERS TO DEVELOP THE SKILLS NEEDED TO LIVE IN THE COMMUNITY. H.O.P.E ENSURES THAT SERVICES AND SUPPORTS ARE IN PLACE TO MEET THE NEEDS OF THE CLIENTS UPON TRANSITION TO THE COMMUNITY. OUR COMMUNITY TEAM CONTINUES TO WORK WITH OUR CLIENTS FOR 18-MONTHS POST-TRANSITION TO ENSURE THAT THEY ARE STABLE AND ABLE TO LIVE SAFELY AND SUCCESSFULLY.IN FY 2025, H.O.P.E. TRANSITIONED 51 CLIENTS FROM NURSING FACILITIES TO THE COMMUNITY. WE WORKED WITH AN ADDITIONAL 552 CLIENTS IN NURSING HOMES TO SKILL BUILD, IDENTIFY FORMAL AND INFORMAL COMMUNITY SUPPORTS AND CREATED SERVICE PLANS WITH GOALS FOR CLIENTS TO WORK ON TO PREPARE FOR TRANSITION. REVIEW TOOLS WILL BE DEVELOPED AND REVISED BY AGEOPTIONS STAFF FOR EACH PROGRAM TO BE MONITORED. PROGRAM PERFORMANCE MONITORING: AGEOPTIONS WILL MONITOR SERVICE PROVIDERS TO DETERMINE THAT THEY ARE OPERATING EFFECTIVELY AND ACCORDING TO APPLICABLE PROGRAM PERFORMANCE STANDARDS.
